Suggest Treatment For Nasal Septum Perforation With Sinusitis

Question: I had a massive sinus infection which was not responding to antibiotics for a month! I was getting surgery for my nose due to fracture and deviated septum from a jaw surgery I got done in my 20'a. The jaw surgery caused post traumatic shock syndrome from the extremity and the pain I endured. I deal with self-harm OCD and I was sent to an ENT to correct fracture and deviated setum and sinuses. I saw fifth week and still a severe infection and he prescribed me steroids and avelox. That was the time I began to self-harm I was so worry of contamination I started to use bleach to clean out my nose and I started to pick areas of my nose with tweezers. I now have 6 holes one right through the septum and any tissue that looked infected I picked out. I also did chemical steams and also used about 12 times a day fucidin acetate to top of my gums which are notw really thin as I wanted the throbbing to stop in my gums. , I have also used in my nose the fucidin acetate and tons of decongestants as my septum is completely collapsed to left side and I need to breath. My nose hasn't collapsed but hole is big and all the other ones are pretty big in size. I have a hole at top of nose so another thing I do as I thought if I put a snort up in my a baking soda paste that would would keep from getting infection. So cat scan from XXXXXXX to September is so hard to believe I could do that much damage. Is this fixable?

In this condition I personally suggest a operation for both condition.

In septal perforation you may go for button surgery and in sinusitis you may go for functional endoscopic sinus surgery after consulting with your doctor.

I have managed to make 6 holes not all on septum ! Can those holes be fixed? I have one on top and sometimes CBF comes out. I am thinking that is very dangerous. I didn't know bleach could do such damage do you think it would cause this much damage? I was just cleaning with q-tip but it was like 8 times a day. My nose is going to be complicated. My deviated septum needs to be fixed but big hole and pretty thinned out but not collapsed. can they fix it in that condition.

Yes, Those holes can be managed

I have gone through your follow up query; and for your information those holes can be managed. There are so many reasons of CSF leak other than bleach if you have.

Better to send the report of CT scan with images of it and images of Nose if possible for further management and /or for your guidance.
