Suggest Treatment For Muscle Sprain In Groin Area

Question: 9 weeks ago I was lifting a 150 lb container over a 2" ledge and into a containment area that was 8" down on the other side. Not doing this particular lift EVER and not thinking prior, I held on to the container too long and felt a shocking sensation at the bottom of both of my testicles. My activity has been very limited since this day and I can feel discomfort daily in both testicles and the cords from my testicles up into my lower abdominal area. Activities such as getting in and out of my car, bathing my kids, putting dishes in the dishwasher, and anything where I bend over seems to aggravate the injury. These activities as well as anything related to that motion have been strongely reduced. My abdominal muscles do not feel any discomfort. I have been to too many doctors all having different opinions. From epditimitis to hernias to damaged nerves with ibuprofen, motrin, hydrocodone, alleve, and an antibiotic recommended I can't get any straight answer with the exception of a strain / tear / sprain in the spermatic cord which seems the most logical. 9 weeks and the injury is no better than it was after 3 weeks. I am scheduled to get a couple of more opinions... hernia has been ruled out by (2) urologists. Thoughts? And if it is a sprain / strain / tear, what can I do to assist the curing table? I have been taking warm baths which was recommended by a urologist.

The persistent pain even after 9 weeks is likely to be due to muscle strain. Lifting a heavy object, twisting, or a sudden movement can cause muscles or ligaments stretch or develop microscopic tears.
TREATMENT: Some general guidelines can help you to reduce the pain. These are as follows:

1) Take adequate bed REST with legs straight. Individuals who do not regularly exercise to build strength and flexibility are more likely to experience such muscle sprains.

2) Alternate Hot water bottle or Ice pack can help to relieve most types of such pain by reducing inflammation.

3) Non-Steroid Anti-Inflammatory Drugs (NSAIDs) are quite useful to reduce inflammation. You can also take oral muscle relaxants. You need proper prescription from your doctor.

4) Exercise : It is a key element of almost any muscle pain treatment plan. You may contact with Physiotherapist regarding this.

It includes 3 components: aerobic conditioning, stretching, and strengthening. The exercises are done through a controlled, progressive program to build more strong and flexible abdominal muscles.

The muscle strain does make the most sense. It encompasses many of the same ailments that I have had in the past with various strains related to sports (hamstring, achilles, ligaments / tendons in elbow, etc). However, my entire area has been affected. My testicles, chords from testicles into lower abdominal area, and even my lower abdominal area are sore and in discomfort all of the time. Getting in / out of the car, and most bending procedures, as well as walking for a period of time increase the discomfort.
Is there anything else it could be or would a strain / pull / tears cause my testicles to continue to be sore, as well as the chords from them into my stomach? Ultrasounds did not show anything wrong or out of whack.
Regarding treatment, you have mentioned to alternate hot water bottle or ice pack to reduce inflammation. Should I be alternating from hot to cold or should I be using just one method hot or cold? Please advise.
I do believe there may be some PT time in the near future. I am seeing another Dr. recommended by a friend on Wednesday. I hope to make some progress here.

Hello.
Thanks for writing again.
The muscle strain can cause a mild tear of the muscle fibres causing a persistent pain. These are not detectable on ultrasound scan. To be more sure and to rule out any other pathology, you can get an MRI scan done.
You can take an oral muscle relaxant and see if the pain subsides. This will help in further confirmation of the muscle related cause.
It is better to use either hot or cold pack- the one which is more comfortable for you.
Start the physiotherapy after the acute pain subsides.
