Suggest Treatment For Multiple Blockages In The Heart Arteries

Question: Hello Doctor, XXXXXXX was admitted to hospital on 04.10.16 and 05.10.16 on doctors have told he has multiple blockage in heart arteries, which can be cause of his feet swelling and cough due to fluid retention. they have suggested open heart surgery. This age is 65, is very week and has chronic cough.

Pls advice what's best for him, as we are worried about post operation care. With heavy cough he might not be able to sustain or recovery properly from the surgery.

I am Dr. Mody and I would be addressing your concern .
Yes multiple blocks in the arteries supplying the heart can cause weaking of the heart which in turn can lead to fluid accumulation and swelling in legs . Chronic cough may be because of pulmonary fibrosis as well as a weak heart .

You are very true that bypass in such cases is risker considering difficulty in weaning of from the ventilator because of his pulmonary fibrosis, however at the same point of time not doing anything will lead to progressive heart failure and will worsen his condition over the period of time .

In such situations it's best to trust your surgeon, if he is confidant of bringing the patient out , we should trust his instinct and allow him do what is best for the patient . He is the one who has examined the patient and knows his history. It would be highly unethical for me to comment of best treatment for the patient who is acutely ill .

You may send me his reports so I get a better insight in his present medical condition and guide you appropriately.
Multivessel angioplasty is also possible based on anatomy and if you can send me the CD of the angiography I can help you with the same , Howe've it toout haso its draw backs and limitationsee in long run , though it would be safer than bypass in acute setting to tide over the crisis
lastly surgery is risky but not impossible in good hands .
