Posted on Mon, 29 Dec 2014
Question: Hi Doctor,
I got a mild wound on my foreskin ,10 days back during intercourse due to insufficient lubricants.I was prescribed to use Mupirocin and the wound is healed now.
Now i feel the following problems.
1.Erection happens in discrete levels(not continous/smooth blood inflow but it rushes in stops again rush in) such that i feel slight pain/sensation at the middle part of my penis.
2.I am unable to maintain erection for more time say 15 minutes,so that by the end of foreplay penis gets down. Also quality if erection is very poor.
3 . Ejaculation happens in 5 to 10 strokes even though i do exercise like kegel.

Brief Answer:
Need physical examination by skin specialist/psychiatrist

Detailed Answer:
Hello,

Thanks for writing to us.

I can understand your concern.

1. You mentioned that "I feel slight pain/sensation at the middle part of my penis".

It suggests some problem is there. If it has occurred after that wound on foreskin, there are two possibilities: Either at same time some injury in penis was also occurred or it may side effect of Mupirocin application and allergic reaction to that drug. For diagnosis proper physical examination is needed by doctor. After examination it may require penile color Doppler to check blood flow.

2. Your second query is regarding your erectile dysfunction. If it has started after above incident, treatment of above is first step to solve this problem.

3. Your third problem can be premature ejaculation (PME). PME is defined as ejaculation occurs before female partner gets orgasm. There is no definition in terms of time or frequency of strokes. If you feel this problem is since long, medicines like clomipramine and paroxetine can help for sure. And other option is to learn Stop & Squeeze technique. You need to consult sexologist/psychiatrist for both approaches.

If after examination your doctor found no reason for erection problem, it is psychological and Sex counseling as well as Master & XXXXXXX technique by sexologist/psychiatrist can help.

Hope |I have answered your query, I will be happy to help further.

Regards,
Dr.Chintan Solanki.

Dear Doctor
W.r. t. first 2 queries : I had already consulted Dermatologist and the skin is perfectly fine and he told it would take some time for curing sensation / pain but he didn't comment on discrete erection.
3.I face PME during 70 % intercourse occasions. I am unable to apply squeeze / stop method as semen comes out automatically without any external force or movements ( within 5 to 10 strokes ) because of the high sensitivity of the skin of the penis and vaginal temperature conditions.
Which is the brand/product name of the medicine you have prescribed?
What is the dosage?
Will it have any adverse effects?

Brief Answer:
You have to consult psychiatrist/sexologist for prescription

Detailed Answer:
Hello,

Thanks for info.

You can wait as per instruction by your doctor for curing of pain and appropriate sensation.

The drug I mentioned are clomipramine (Anafranil or Clofranil) and paroxetine(paxil). Generally it is taken in 10 mg dose (for both dose is same) at night daily.
Clomipramine in 10 mg is probable without any significant side effect. With paxil you may have some gastric discomfort and headache.
If I were your treating doctor, I would like to start with clomipramine. After improvement this medicine should be taken before sex only.

Any medicine must be taken after consulting doctor and written prescription. So if any adverse reaction occurs your doctor can help you.
I am sorry that online prescription is not at all valid according to law. You must consult psychiatrist/qualified sexologist for the same as medicines mentioned by me are prescription drugs and won’t be available without prescription.

If still any query, I will be glad answer.
