Suggest Treatment For Mild Fever, Blocked Nose And Cough

Question: Hi,
My 2.5 year son had got mild fever , blocked nose and very little cough 6 days back . Also he was scratching his ear a bit so doctor siad it's a mild ear infection . Doctor suggested to give ibugesic+ for fever whenever it came , sinarest for blocked nose and started with moxclav antibiotic for 6 days.
Fever wasn't much and was ranging between 98-100 and was coming in duration of 8-12 hrs and disappeared in 3-4 days.
Blocked nose and cold got better in another 3-4 days ,and also dint see the little one scratching his ears in last 3-4 days.
Now since day before yesterday he got mild cough in night and it increased a little more in morning so we started with DILO syrup but by evening the cough had increased a lot with the little one coughing non stop every 15-30 seconds. Also he developed a continuous runny nose since yesterday morning.
He doesn't not cough while sleeping but coughs a lot while awake. We have tried everything from honey , steam , black tea , small pieces of koflet lozenges/vicks etc but nothing has really worked so far and the duration of cough since yesterday has only increased slightly with no fever since two days. We are currently giving him dilo three times a day and completing the 6 day course of antibiotic moxclav. There have been few occasions where the duration of cough has reduced slightly but overall he continues to cough non stop even while sitting / playing /talking or watching videos. We noticed that when he talks less his cough duration goes down but if he keeps talking he continues to cough continuously .

We are worried with the increase in cough since yesterday as despite antibiotics cough has increased from being very mild 6 days back to very high now.
We are currently giving dilo since yesterday 3ml thrice a day , Moxclav 5 ml twice a day and sinarest 3ml twice a day.
I have few questions ?
1)He does not cough while sleeping and coughs only when awake what could be the reason , he does not look short of breath at all.
2) could it be pneumonia ? But there is no fever since 2 days
3) does steam works and if yes what should be the duration ?
4)he has been eating a lot of sugar since 3-4 days , can it increase cough to this level .
5) weather has changed since 1.5 days and has become slightly colder in XXXXXXX It started raining since yesterday evening after which the cough really increased.
6)Also wanted to know can we give lozenges like koflet and Vicks candies after crushing them to small pieces?
7) Any suggestion to stop coughing immediately

Would be thankful if you could answer all the questions in detail and your suggestions . Looking forward to your reply.

I can understand your concern.I have gone through the detail history and I have concluded that your child is suffering from upper respiratory tract infection may be viral or atypical bacterial.

Severe persistent cough in absence of fever can be due to infection with atypical bacteria like mycoplasma.The other possibility is viral infection or pertussis.

You have mentioned that you are giving Dilo,moxcalv and sinarest.
I want to know which syrup you are using Dilo BM or Dilo Dx.The composting of both the syrup are different one contains bronchodilator and other cough suppressant.
Moxcalv is an antibiotic.
Sinarest contains decongestant and antiallergic.
There is no harm in giving cough lozenges but as I think you already aware of is that there is chance of chocking.
Be cautious that before giving you have crushed them well.
You can give steam inhalation for 15-20 minutes, 4-5 times a day.
I really appreciate your observational skill as you have mentioned that there is no breathing difficulty.
I agree,if there is no increase in respiratory rate or retraction and fever is absent chances of pneumonia is very less.

Here I would like to give azithromycin antibiotic which has a good coverage for both atypical organism and pertussis.
Also tell me which cough syrup you are using Dilo Bm or Dilo Dx,as I would like to add cough suppressant to get relieved from cough.

Possibility of precipitation of cough due to cold could not be ruled out.So better you keep the room temperature around 28-30 Celsius.

Please provide me details so that I better able to help you out.

We are giving dilo bm .
Should we give steam with Vicks /eucalyptus oil roust plain water.
If it's viral/pertussis by when can we see improvement in cough duration. Although at times it does reduce.

Also what is the reason for coughing while talking and no coughing while sleeping or less coughing while not talking to others.

Earlier on several occasions he had cough bouts and may be he is allergic to dust etc , he has responded well to montair lc , but this time the cough duration is slightly more with increased coughing. I forgot to mention his runny nose has a white colourless discharge3-4 days back it was yellow which has turned white colorless now

If there is a need for a allergy test how is that done in small kids. Also is there a need for a blood test as of now. Wanted to know how is blood test done in small kids is it the normal way done in adults or is there another way using a pin or something used for plucking blood drops etc.

Giving steam with Vicks/eucalyptus oil will provide some relief in both cough and congestion.
Cough receptors are present in the airways from posterior part of mouth (pharynx, larynx) to the lower airways. Sensitivity to these cough receptor increases following infection or allergy. The heightened cough receptor sensitivity can cause cough even during talking or laughing. The same is happening in your child.
In most of the cases when infection is limited to upper airway the cough usually subsides within 10-14 days (depending on the severity).
Montair LC contains long (montelukast) and short (levocetrizine) term controller of allergy.
As the sinarest you are giving also contains antiallergic (chlorpheniramine) there is no need of Montair LC presently.
If there is past history of cough which responded to Montair LC it suggests that there is some allergic component too. So continue with Sinarest, Dilo BM and steam.

As there is no fever I don't think that investigation would help much. The investigations may be needed if cough persists for longer duration, in presence of respiratory distress or if fever recurs.
The process of blood sampling is same as in adult(by vaccutainer/syringe with needle).
I don't think that he needs allergy test presently. Allergy tests are done by skin(patch test/prick test) or blood test.Response to a variety of stimulus (allergens) is noted in allergy test.
I would like to add Azithromycin to cover the atypical organism and pertussis infection. This is a prescription drug so please get it prescribed by a physician. Continue Dilo BM and Sinarest.
If condition worsens consult your physician for clinical evaluation and management accordingly.
