Suggest Treatment For Lymph Node TB

Question: My Wife is diagnosed with Lymph node TB and currently she is undergoing TB treatment for the past 2 months in XXXXXXX However , swelling in the lymph node is not subsiding at all so far.
We visited again one of the TB specialist in our hometown (haryana) and he informed that the medication which are given currently are of less mg (700-800) whereas he advised 1200 mg medicine based upon his extensive experience with this disease.
Pls advise!

Thanks for your question. I can understand your situation and problem.

Treatment of tuberculosis is based on weight. All drugs should be given according to the weight, so that under dosing or over dosing can be avoided.
So I need to know few things first.
1. What is her weight at present and at the time of treatment initiation?
2. Is she taking AKT4 simultaneously (means taking all 4 tablets at a time)
3. Is she having cervical lymphnode tuberculosis? As you have not mentioned site of lymph nodes in your description.
4. Please upload all the investigation reports, you got it done till now for this concern.

1. What is her weight at present and at the time of treatment initiation?
Earlier it was 48.5 kg and now its 47.8 kg. But it can be attributed to the recent loads of exertion happening as she joined office on 1st decemeber after 7 months of maternity leave. It involves twice to & fro travel to office as she comes at home during lunch to feed the baby. And other work related stress also adding to that.

2. Is she taking AKT4 simultaneously (means taking all 4 tablets at a time)
No. Its taken 1 empty stomach, 1 After breakfast & benadon, 1 After lunch and 1 post dinner.

3. Is she having cervical lymphnode tuberculosis? As you have not mentioned site of lymph nodes in your description.

4. Please upload all the investigation reports, you got it done till now for this concern.

Hello dear, thanks for your follow up question.
Thanks for the information. I have gone through all the reports.
I think she should take whole AKT4 pack (4 tablets) together.
Because this is the way it should be taken, not the way she is taking.
AKT4 has 4 anti-TB drugs, with different mechanisms of action. So all the drugs need to be taken together, so that maximum (peak) effect can be achieved.
This is the ideal way to take anti TB drugs.
And AKT4 is sufficient according to her weight.

I am surprised how come a doctor with 15 + years of experience hasnt prescribed ideal way to take up the medicine.

As she has been taking medicine almost for the past two months per previously stated schedule, is it possible/good/safe to start taking up the way you advised?

Could you also advise us any experienced Pulmonogist whom we can visit and carry forward the treatment as i have lost trust on the current doc.

I can understand your situation and problem. There is a lot of malpractice done in prescribing anti TB drugs.
I advice you to continue AKT4 , all 4 drugs at a time. Best is on empty stomach, but she can take after light breakfast. Continue benadone tablet. Take these for 1 month. Any if improvement is not seen then get done biopsy.
Had she undergone biopsy? As in your reports only FNAC report is available. No biopsy report. And where are you living. I mean in which city?
