This is definitely unusual to have such a disparity.
Since the swelling is for several years, the commonest cause of non-reducing lump in scrotum is hydrocele unless proved otherwise.
I would advise you the following:
Consult your Doctor, preferably a Surgeon for clinical evaluation. Simple trans-illumination tests with the help of a pointed torch in a dark room gives the diagnosis, the light passes through and gives a glow in hydrocele.
If this is not present an ultrasonography of the scrotum will give a proper diagnosis.

This is hard to believe as there would have been a definite diagnosis if the swelling was present that time too.
As said earlier, simple trans-illumination tests and ultrasound should help to get a diagnosis.
If the swelling does not reduce meaning is constant, it will show up on clinical examination and ultrasound.
If this is reducible and regresses and enlarges, then this may be a hernia and if ultrasonography is done in the reduced state, it will be negative.
Please get a fresh ultrasound.
