Suggest Treatment For Lower Back Pain

Question: I have low back pain above hips from 6-7 me months which increases while sitting walking n heat eases the pain. I did MRI screening of back which slowed disc bulges in C5 C6 L2 L3 L 4 L5 levels indenting the thecal sac without neural compression. x ray was normal. S.I. MRI screening is normal. Blood test of RA factor, HLA B 27 PLC , ESR, CRP and PLC all r normal. After medication physiotherapy swimming exer...nothing is working plz guide

I'm Dr. Sudhir Kumar,

After going through your query I concluded that you are having chronic backache. Your MRI has been done already. It shows disc bulges with indenting the thecal sac at L4-L5 level. This may be cause of back pain.

Treatment of it is rest in position of relief (generally lateral position with both hip and knee bend), neurotopics such as mecobalamin and analgesics (Diclofenac three times a day after meals is effective). You can take omeprazole before meals to prevent acidity caused by analgesics. You are already taking vitaminD3 and calcium. You can continue with them. I will like to see your vitamin D3 level as its deficiency is important cause of backache. So get vitaminD3 level done, however you are already taking vitamin D3. You can discuss with your treating Doctor about it.

Thank you doctor. My D 3 level was 9 few months back...in this year I have taken two months supplement of d3. Can d3 deficiency cause this much pain. For how long should I continue d3 supplements. I am on leave from work from past 7 months n just do some household work at my ease.I take d3 XXXXXXX once a week n calcium level was 9.8 should I continue those tablets also. Can d3 deficiency cause stiffness in fingers also after rest.

After going through your query and follow up query, I want to say that your vitamin D3 level was very low.Vitamin D3 deficiency can cause unexplained pain of back and joints. So vitamin D3 deficiency can cause so much pain. It can also cause finger joint pains.

Course of vitamin D3 is 60000 IU weekly for 10 weeks. After that you can repeat the test and if it is still low then you can continue for another course. Otherwise you can stop vitamin D3. If you have taken full dose supplements (60000 IU per week) for two months then you can take it for one more month and repeat the test.

You are having some lumbar disc prolapse which may be also cause of pain.

Rest has already been discussed. You can discuss with your Doctor about it.

Welcome back

After going through your query and follow up query, I want to say that normal level of vitamin D is 30-100 ng/ml. I think you should stop taking supplements when you achieve level between 60-80ng/ml however minimum level of D3 so that no supplement is required is 30 ng/ml. There are no known benefits to having a vitamin D level over 100 ng/ml. Level above 150ng/ml are considered toxic.

Sunlight between 11AM and 4PM with skin surfaces exposed (body exposed) is required to synthesis natural vitamin D3. Morning and evening sunlight without body exposure is not of much use.
