Suggest Treatment For Lower Back Pain After A Fall

Question: Hi there,
I fell on my bump about 1.5 months ago and only had lower back pains for two weeks.
When I felt better I went to the gym and felt ill after doing a few repetitions on the bench-press.

The next morning I felt passing out in the shower and rushed to the hospital where they did a CT scan and concluded I have a concussion.

My symptoms are head pains / heat in specific areas, blurry vision and balance issues.
I also feel like my brain is going to explode every time I take a shower and the pain lasts for about 6 hours.

I saw a neurologist at my local hospital who said I don't have a concussion but Occipital Neuralgia.

Please help!
I am confused and need direction on the treatment needed to get back to being healthy again.

I read your query and understand your concerns.

I endorse opinion of your neurologist on the following grounds regarding absence of concussion.

First there was a period of normalcy for two weeks. It is very unusual for a person with concussion.

Second the fall was on bump mean no direct injury over head.

Third till date NO CT scan is enough to say whether concussion is present or not provided there is no major damage which is unlikely considering two weeks of normalcy. The MRI report is the minimal requirement to say whether concussion is present or not.

Finally the wosening of symptoms after going to GYM indicates there is something else other than the diagnosis.

Occipital Neuralgia is possibility as there are balance issues along with headache and symptoms are worsened after having a shower. Blurry vision is also a symptom of occipital neuralgia.

In my view follow the instruction of your neurologist before searching anything else as he the right person to treat you accordingly.

In addition if you feel there is still something wrong upload your MRI report for further evaluation and guidance.

Regarding X ray lumbar spine I do not feel requirement for same because you have normal activity for two weeks and thereafter. In addition none of the current symptoms can not be explained on the basis of back injury.

I saw another doctor today and asked him to be my family doctor as I do not have one.

He was very thorough with his questions and came to the conclusion that I do have an occipital neuralgia.

When I asked for his referral for an MRI he refused and only prescribed Tylenol, massage therapy and rest.

As of today I can feel my head becoming some sensitive and I get small needle pain sensation in the back of my head when I lay down.

Do you have any recommendation to treat these symptoms?

I am glad to know that you got your family doctor and he is very upto date with all through approach.

Although I think in absence of neuralgia Tylenol is enough medication but in case it does not help completely ask your doctor to prescribe low dose of Amitryptiline. This will help in pain as well as sensitivity. In addition it will reduce your stress which is going on gor last six weeks.

I hope he is convinced with the absence of concussion which was diagnosed around 4 weeks back.
