Question: Sir, my aunt`s mother is having blood sugar from 15 yrs and it is 230 right now. when she went to the doctor she had been told that she had developed a cyst in lung and was told to get ct-scan and endoscopy done but her mother`s heamoglobin was 8.8 which should be atleast 10 for tests. So she has been called the next month. please suggest what she should do. i would be very thankful.

Hemoglobin is required for good health. A level of below 10 means your aunts mother has anemia. There is no problem in doing a CT scan to see the cyst.

However, for endoscopy, blood hemoglobin levels should be more than 10 to avoid any serious complications if there is bleeding during the procedure. This risk is 1 percent and therefore the doctor does not want to take a chance.

She should eat nutritious food and also take fruits, proteins and medications to increase blood hemoglobin.

Meanwhile she can get the CT scan done if possible. If hemoglobin is 8.8 then there is no additional risk in doing CT scan.

Sir,
Although i got all ur answers but one thing i want to enquire,
it is that doctors said my aunt about her mother`s serious condition and told that her life might not be too long enough. if cyst is found is it going to harm her because what doctor`s told her was that the cyst might take the form of cancer. Is the high blood sugar level also harmful?

Sugar level of 230 is high and this can affect kidneys, eyes and nerves in patients having diabetes.

Please tell her to get kidney function and eyes to be tested at least once a year.

my aunt has emailed me the following details, which can u please say about,

We have got the Xray done !
Is city scan important ?
She is taking Hem up syrup !
But her Hemoglobin is fluctuating !
I feel she is week for Endoscopy !
She got done 2 years back & she had small small ulcers at that time ! We gave her Mucaine syrup & Syscan Tablets !
She can not walk up to the Hospital !
Can we not give her medicine on the basis of Xray ! Earlier there was no city scans !

My observations are as below
1. Since I am expert in X ray and CT scan, I want to see the X ray report and image if possible and when it was done.

2. Unless you share details of X ray I will not be able to say what exactly is the situation.

3. Hemoglobin can be improved by eating good food and taking tonic Haem up.

4. A repeat endoscopy is required to see the current position of ulcers in stomach.

5. She can be given mucaine for ulcer. Foods must be given cooked in less spices and oil. Frequent small meals might be better.

6. Syscan is fluconazole an antifungal medication and should be given only if required. Giving it without doing endoscopy can have other side effects if she was prescribed that for fungal oesophagitis.

Sir,
my aunt said that she would send me the X-ray and city scan reports later on which i will forward to you later on. she has told that she would not go for endoscopy and would go for city scan next week. she has sent me following details:
My mother has been taking excellent diet for last 3 years! Still Hemoglobin went down! Doctors are worried!
Last time her plight became very bad after endoscopy
I will go for city scan within 7 days but no to endoscopy ! She got sick last time

My observations are as given below
1. I shall wait for X ray and CT reports from you.

2. Endoscopy can be uncomfortable in many patients. However with new endoscopes have latest fiberoptic technology and are available in thinner sizes. In good hands, doing an endoscopy should not be a problem.

3. There can be many causes for hemoglobin levels not rising even after taking good and nutritious foods. This can be due to many reasons like decreased absorption of nutrients from the intestines, decreased generation of red blood cells or increased destruction of red blood cells. To know the type of anemia she might be having it is important to do a blood test called peripheral blood smear. This is a simple investigation and only two drops of blood are required.

4. You can update me when you get the reports after doing CT scan in 7 days. Please also send me old reports for comparison if you have them.
