Question: 82 year old male wth blood pressure that drops to 50 twice daily, dizzaness,swellng in feet, confusion at times, constant UTI and has been in acute delerium however not at present

You mentioned that BP falls to 50 . What I need to know is whether its systolic or diastolic BP ? Because a systolic BP of 50 means he is in shock.
Dizziness could be due to many reasons. But low BP can explain dizziness. Other causes that needs to be evaluated is his blood sugars blood sodium levels. Kindly get these blood tests done.
Altered behavior at his age could be due to UTI or infection at other part of his body most likely his lungs. I advise to get chest X ray blood culture sensitivity test and urine culture sensitivity test done.
Dementia is a possibility but should be considered after the above causes have been tested and ruled out . If all the above tests have been done and if they come out normal then a MRI brain and CSF analysis has to be done.
