Suggest Treatment For Light-headednesses And High BP

Question: Hi I have been suffering with lightheadnesses with movement for a number of years now that My GP and I are contributing to Peri Menopause but he has told me to just live with it...... it has been getting worse this last week or so.... I do have High blood pressure and on coverram for it.... one thing that has been checked is blood pressure dropping when moving but no it goes up I have a machine at home and today for eg sitting it was 130/77 then I took it standing and got 145/90 what can it be that is causing this?. and could that be the reason for my lightheadnesses I have terrible headaches as well..

I have had MRI and CT and heart tests and blood tests and seen a nurologist and eliminated vertigo and other major issues.

also the lightheadness is fine when I am lying or sitting down its only when I am walking or standing still sort of like motion sickness on the land could it all be hormonal or any other reasons and is there anything I can do to fix the BP rasing so much on movement

This is Dr.Riyanka and I am glad to be helping you with your query.

Firstly your MRI,CT, cardiovascular functions and blood tests are normal so we can exclude any organic diseases.

Lightheadedness can be attributed to high BP which can occur if you are not maintainingyour BP under control. Tablet coverram contains perindopri and amlodipine so this is apt for your blood pressure maintainance.

I would like to comment on your statement "eg sitting it was 130/77 then I took it standing and got 145/90 ". Now, BP is definitely influenced by posture and usually there is a drop in systolic blood pressure(numerator reading) when a person shifts position from lying down/sitting to standing because of venous pooling in of blood to the lower extremities.

I suggest kindly visit a doctor who can take a note of your BP readings for three different occassions in a day - morning , afternoon and evening. Make sure you take the readings in supine (lying down posture after 10 minutes of rest) and immediately within 15 seconds of standing . This test will help exclude any postural hypotension and autonomic dysfunction.

Also whenever you measure BP , make sure the BP instrument is at the level of your heart regardless of the position in which you are measuring the BP so that you get appropriate results without any errors.

Kindly also get Autonomic Function tests to exclude any autonomic disorders that could be causing BP fluctuations leading to light headedness.

Terrible headaches could be either due to your BP shooting up in which case your dose of medications need to be adjusted and also you need diet control. please regulate the salt content in your diet. Other causes could be migraine for which you need to take analgesic medications and you need to avoid triggering factors like perfumes, scented candles, chocolates, exposure to extreme sunlight .

Hormonal imbalance can cause lightheadedness , irritability and flushing. If the symptoms are very sever then a consultation with the gynaecologist can be recommended for Hormone Replacement Therapy. Also, as i suggested please get Autonomic function tests done along with Blood Glucose (fasting and post prandial ) to rule out diabetes.
