Suggest Treatment For Knee And Severe Joints Pain In A Teenager

Question: I have a 17 yr old daughter who has experienced knee pain on and off since she was about 5. For years was told my doctors it was growing pains. After age 15 I could no longer accept that diagnosis, she did physical therapy to strengthen the muscles in her knees and thighs, and has had her knees taped and so on and so forth. This past mother's day a week after having a mild cold she developed SEVERE joint pain in her legs, (ankles up to hips) and in her hands and time to time her elbows and toes. She also has a frequent headache in the back of her neck. She has seen her GP which first diagnosed her with anemia and put her on Iron pills (325 mg daily) and said she had a severe Vitiman D deficiency and put her on 50,000 IU 1x weekly. He recommended we see a rheumotologist which we did and she did a thorough exam and blood tests. She ruled out any type of arthritis or parvovirus, but felt that it might just be a reaction caused by a virus that occurred over mother's day and said to give it about 6+ weeks. It's been much longer than that time period and her pain has never left, she has tolerated it but in the last 24 hours it has gone back to the severity it was when it all first happened. I don't know what type of doctor to take her to next. Her current meds are Vitamin D3 50,000 1x weekly, Lo loestren (birth control, which she was put on for VERY painful periods and it has worked), Iron pills 325 mg 1x daily, and Zoloft 100mg for generalized depression and anxiety. Can you give me any ideas as to where to go to get her help? Narcotics and general pain relievers and muscle relaxants don't do anything. Her joints are NOT red, not swolled, are not warm/hot, usually her hands and feet are very cold.

I understand your concern..

I think there are many other causes of knee pain in young people which need to be rule out...

There are many soft tissue malformation which can leads to knee pain.. A vigilant clinical exam is necessary is necessary...

Hyper-mobile joint, flat foot, maltraction of patella etc are important differential...discuss with your doctor

Xray knee merchant view and repeat clinical exam is necessary...

vitamin D deficiency is very common but it is unusual to cause such type of pain.

knee physiotherapy is also considered are clinical exam

for anxiety, please discuss with your doctor to tryptomer, this is excellent to treat muscular knee pain also

Thank you for your response Dr. Alokkalani, however your response did not cover all my daughter's issues. It's not only her knees that are giving her pain. As i stated in my notes her joints, hands, finger, toes, ankles etc hurt. Her discription is that it is not muscular but that it is bone....she says it feels like the bones are trying to push out of her skin. I don't know what type of doctor to take her too. We are seeing her GP on Monday as well now as a neurologist. Is there anything you can think of that could be causing all this pain...it is NOT arthritus as that has already be ruled out by a rhuematologist so has a parvo virus, rheumatic fever, mono and strep.

I understand that ..
hypermobile joints can cause pain in almost every joints..
maltraction of patella and flat foot both are important knee pain differential diagnosis and chronic knee pain leads to psychological pain else where also. This is my personal experience. Fibromyalgia is another differential diagnosis.

I advice you to see your rheumatologist and ask whether it is Fibromyalgia or hypermobile joints?

I donot think that neurologist can deals with joint pain..
