Suggest Treatment For Kidney Stones

Question: Hi ! My sister has been detected with calcium stones in middle calyx, 4.5 mm in right kidney and 7.2 mm in left kidney. She is of 26 yrs, weight 65 Kg. She is having mild pain/nausea few times. How serious is that and please suggest on treatment, cost, and time it will take to recover? Also,if you can suggest hospital/doctor in Delhi/Bangalore for treatment.

Thanks for your query,based on the facts that you have posted it appears that your sister has been detected to have bilateral Kidney stones .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Does she have severe pain in loin?
2) Is there any obstructive changes (Hydronephrosis) reported in her Ultrasound Scanning report ?

The primary aim in management of kidney stone is to save kidney from damage resulting from back pressure changes when the stone gets impacted in ureter (A tube that carries urine form kidney ) in to the bladder)

I would suggest following course of action for your sister
1) Rt side stone is small in size 4.5 mm and non obstructing in nature hence likely to pass down spontaneously and does not need any active treatment .

As regards stone in the left kidney it being of the size of 7.2 mm can also be expected to pass down spontaneously but may cause severe pain if gets impacted while passing down .Hence there are following two options for treatment

1) You can fragment the stone by what is called as Extra Corporeal Shock Wave Lithotripsy(E.S.W.L). This is a noninvasive procedure done as a out patient and does not require anesthesia or Hospitalization and cost about 15-20 Thousand Rupees

2) Second alternative is to wait and watch and expect it to pass out spontaneously
However if she develops severe pain and vomiting during the course of expectant treatment you should be prepared to go for endoscopic procedure called URS to fragment the stone which requires anesthesia and hospitalization for 3 days and will cost 40-50 thousand Rupees.

You can give her following medications
1) Antibiotic --Norfloxacin twice daily
2) Antispasmodics - Cyclopam twice daily
3) Ureter smooth muscle relaxant -Tomsulosin once daily
4) Ask her to drink more water to produce more urine

Take the decision as per your choice

Thank you Sir for the detailed answer. Have attached ultrasound, blood & urine test reports. My sister is currently in XXXXXXX and have some exams lined up in next 3 weeks. So, we are planning to take the treatment in bangalore/delhi by 4th week of XXXXXXX She is having moments of pain once or two in a day and no vomitting. Currently she is taking -
1) Nilstone Syrup
2) Nilstone potent litholytic
3) Drotin A6 Uripas

Kindly suggest if there is any immediate need for ESWL. Also, if current medication is lacking in any way.
