Question: 83 yr female with kidney failure has indwelling XXXXXXX bed bound hx lupus,htn, cad, end stage alz with dementia anorexia poor fluid intake, now has lg amt yeastydrainage from urethra, has been on diflucan x3doses,also monistat only c/o pain when drainage comes out around XXXXXXX deainage is yellow/green occ has cottage texture what can be the cause

Hi,

Thank you for your query. I can understand your concerns.
Your aunt is having acute urethritis.
Yellowish green discharge indicates bacterial infection likely pseudomonas infection.
You need to subject urine for routine analysis and Urine for culture and sensitivity.
Antibiotic and dosage would be determined by culture sensitivity report.

You need to continue one oral anti-fungal drug ( e.g. Voriconazole).

Till urine report including cultures are available, one can start first line drug (e.g. Nitrofurantoin 100 mg three times a day orally) after discussing with your primary doctor.

she only takes approx.4-5oz of fluids, hold food and meds. in her mouth so at her age would IV or IM meds be better for her. she is being followed by hospice. if condition is not treated
,will cond worsen

First line drugs are oral and can be swallowed with fluid (drug can be dissolved in water and given in sips). Second line drugs (as indicated by urine culture report) may be given by IM or IV.
