Question: I have a red rash around my anus area that itches badly. I did strain a few days ago to have a bowel movement and I noticed it immediately afterwards. I have also been experiencing hot flashes and I thought it might be just a rash due to sweating but I'm just not sure.

I am so sorry to hear about this rash that you have reported, I have reviewed the image and it is not typical of any specific infection. it looks more like rash caused by irritation and not a specific germ. At this time, I will like to recommend what we call sitz baths. Should this fail to resolve your symptoms and the rash, then we would the consider further examination and testing.

To do sitz baths, add 20ml of Iodine to 1.5L of lukewarm water in a bowl of lukewarm water. Sit in this for 15mins at least thrice a day and after every stool. This would help with the itches and accelerate healing. For now, i will recommend you hold back with all creams and powders. I will recommend you dress light and use wipes to keep this area dry, airy and not humid.

Report your progress within a 3-5days and we would review to see what the next measures to take are.

The sitz bath does not seem to be helping at all. It continues to itch and does not look tobe getting better but remains the same. It makes it especially difficult to sleep at night. Is there anything else I can try?

Hi and thanks for this follow up.

I am so sorry to hear about this persistence of itches and the difficulties sleeping. I will want to treat for possible fungal infection at this time as the itches seem not to be subsiding. When you mentioned this occurring immediately after straining during stools, it made the chances of this being an infection very slim. However, with persistence of symptoms, I will want to treat for the most common cause of itchy infection in this area which are fungal infections.

Apply cotrimazole cream, twice daily for 10days. For the itches, use benadryl for relief. All the measures above except sitz baths still apply.
