Brief Answer:
Self care & needful skin specialist's advise.

Detailed Answer:
Hi XXXX,
Thanks for the query under Health care magic.I have read it & understood your concern.
Itching scalp with bumps & hair loss can be due to-
- Allergic reaction to shampoo or hair oil, conditioner, creme.
- Fungal infection ( dandruff) of hair roots.
- Lice infestation
All of these cause intense itching - thus damaging skin which can get secondarily infected with bacteria. Thus folliculitis of scalp occurs .. causing the bumps. This also affects hair roots which get dislodged - hair loss.
* Home care- a deep observation can find the possible cause & needful care of hair can minimize & control the problem most of the times.
- Dandruff- Appears as white flakes of dead skin in hairs.
Home management consists of frequent shampooing with plain shampoo/ anti dandruff shampoo ( with Ketakonezole ).
- Lice infestation combing with special lice comb one can see live XXXXXXX lice / it's eggs stuck to hair.
Home management consists of frequent combing & removing lice along with anti lice shampoo every three days till they are totally irradiated
- Allergic cause can be detected with start of symptoms with use of the causative hair care product.
Management is by stopping the use of allergic product & taking oral anti allergic pills for few days.
* Doctor's advise- When there is no relief with home management , a skin specialist should be consulted. he would add few needful medication for sturdy cases.
* Hair care-
- self care- Habit of Daily hair combing with 100 strokes at least on each side/ weekly or biweekly hair bathing with hair root massage with oil the previous night/ avoiding exposure to extreme temperature.
- Doctor would also advise needful for needful hair tonic helping hair health ( Containing vitamin C,A/ Omega 3 fatty acid/ protein/ iron/ celenium, biotin, zink )
Thus self care & needful doctor's advise & routine hair care would not only relieve the present problem but also would keep hairs healthy & prevent recurrence .
I hope that answers your query. Any follow up query is welcome.
Thank you.

Brief Answer:
A dermatologist doctor's advise will help.

Detailed Answer:
Hi XXXX,
Thank you for the follow up query & added information about the lesion.
a) The information about the history of presence of plaques getting oily/ wet after scratching .. brings the possibility of Seborrhoeic dermatitis .
It is a condition of itchy scalp rash along with irritation of sebaceous glands in scalp skin.. which secretes more oil.flaky scales of dead cells & hairs stick together to form what you call plaques. Secondary infection by fungus or yeast or bacteria can follow later. In severe cases this leads to hair fall.
b) Any of the cause explained in previous answer. can irritate the sebaceous glands in scalp to have same effect..
* The management of the scalp itching with oily skin ( due to overactive sebaceous glands )differs in few ways.
- Removal of the hard plaques is essential.. so that the medicine reaches the skin & have relief. This is not to be done forcefully by pulling/ scratching. Coal tar cream ( or any other soother as per dermatologist's advise )should be applied to the plaques for several hours & then washed with the shampoo.
- At least twice a week shampooing with Ketaconazole shampoo( Or any other shampoo your dermatologist advises) till there is complete cure.
- Steroid ointments help to relieve itching.
- A regular hair care schedule to be followed using anti dandruff shampoo & non greasy hair conditioners.
* You should consult an expert Dermatologist only for expert advise as all above said tips need to be specific after actual examination.
I hope that helps you. Any other query you have please feel free to ask.
Thank you.
