He need further evaluation.Contrast CT scan of scan of abdomen should be done for further evaluation.Contrast scan can show its enhancement, vacularity etc.It can also show other associated findings like lymph nodes etc.
Since he is an old aged person,it is important to rule out malignancy.

Hello XXXX
As we discussed earlier,it is too early to conclude about cancer or metastasis.
Many features can't be evaluated by USG.
Contrast CT scan(Abdomen),tumour markers,FNAC ,Clinical correlation confirms malignancy.
If it is due to metastasis, primary source should be searched.
Your grand father need proper clinical evaluation.Chest X-ray(PA),Liver function tests should also be done.
Liver cancer causes weight loss, pain, jaundice etc.
You shouldn't be so worried. Please go for further investigations.
Treatment depend upon findings.
Your grand father also has calculus in right kidney and urinary bladder. You should also consult urologist.
Get well soon.
Hope I have answered your question.
