Question: Hello Doctor,

I am married. I have a problem with irregular menstruation. It skips for at times 2 months at a stretch. I recently got married but we never had intercourse so far. We are only having orals like fingering. 10 days back, I and my husband just had orals at night; same day morning I had a little crampy pain in abdomen which lasted for couple of hours and then went off. Around 5 days back I started having constant cramps in my lower abdomen which were extreme for 1-2 days n then subsided to minimal with a feeling of pain in lower abdomen. I haven't got periods yet. Is the pain due to periods not coming, if yes please tell me which medicine to start with to get periods in coming days.

Hello,

Thanks for writing to us. Followings are my comments:

1) Above features may be related with premenstrual symptoms (PMS) and it indicates that you will get period shortly. I suggest to wait few more days to get your period naturally.

2) If you have irregular period AFTER marriage, then it could be due to hormonal imbalance, stress, change in weight or diet pattern.

If you don't get your period within this week, I can suggest Norethisterone tab (like PRIMOLUT-N) for 5 days to induce period. You will get withdrawal bleeding like period within 5-7 days after stopping them.

3) If you have the problem from very beginning and BEFORE marriage with skipping period for 2 months or more, we need to rule out POLY CYSTIC OVARIAN DISEASE in this context.

Relevant investigations are required to carry out after clinical evaluation. These include Pelvic ultrasound scan (USG), Serum prolactin level & Hb%, complete hormone test - LH/ FSH, thyroid profile etc.

In that case, Combined hormonal pill will be helpful to regulate your monthly period and you will get more than 99% protection against unwanted pregnancy after 1-month used.

Kindly, consult with local gynecologist for above mentioned tests and procure complete prescription.

PCOS was done. It was negative.

Rest of the tests need to be done. Will get it done shortly.

I have irregular menstruation since a long time.

One more thing I would like to ask you is that, is it possible that the precum can play any role in pregnancy like if there is slight precum on the penis and its just rubbed over the outer wall of the vagina, can it induce pregnancy and if yes under what circumstances? [Ectopic pregnancy any chance?]

Also want to ask you why there is constant mild pain in lower abdomen? I suddenly started with the pain; first 3 days it was very severe but now its mild and constant. Earlier when I used to have the feeling of onset of periods, I used to get cramps but not the constant types. Just would last for couple of hours or a day and then go off. This time why it is constant?

Also a general question, after how many days a girl will show signs or symptoms of pregnancy?

Precum rarely causes pregnancy in your case.

Hello,

Thanks for follow up query.

1) Precum (contains sperm less than 10%) RARELY causes pregnancy in your case. You should know that pregnancy depends on proper timing of ovulation and unprotected sexual intercourse with ejaculation inside vagina.

2) Rubbing penis on outer part of vagina or labia DOESN'T get you pregnant. If you have any doubt, you can undergo one home pregnancy test.

3) Abdominal bloating or cramp may be due to indigestion problem or late period itself. Pelvic infection, change in weight, stress can precipitate the same. Kindly, consult with your doctor regarding this.

I suggest you to take Drotaverine tab to reduce abdominal cramp. Take easy digestible food, drink more water, be stress free and take sound sleep. It is a prescription medication.

1) Above said medicine can be taken twice daily for 5 days to induce period. You will get withdrawal bleeding within 5-7 days after stopping them.

2) Women can feel pregnancy symptoms soon after missed period or within 1-2 weeks (max).

I will wait for couple of days and then start with Primolut N. What should be the dose 5 mg or 10mg? Should I take it before lunch and dinner or after lunch and dinner?
