Suggest Treatment For Irregular Heart Beats

Posted on Thu, 15 Jun 2017
Question: I have had sticky palms and bottoms of feet since the end of March. I am a 46 year old female. After about 4 weeks they started sweating now not as bad, but I am also now very sweaty in my private areas-as in my whole pair of shorts will be wet, as well as behind my knees. I thought maybe it was hyperhydrosis but I don't know. My MD did give Drysol on Tuesday, not working yet, and I think I over-researched my m and walked in and said I think I have this.
I take a number of meds. I am a juvenile diabetic and a celiac (new diagnosis). I need help, I need to get back to work but feel so gross.

Brief Answer:
About excessive sweating;

Detailed Answer:
Hello and thank you for asking!
I can understand your concern.
I think hyperthyroidism and other patholo logins causing excess sweating (TB, lymphoma ect).

If along with sweating, you have bladder control problems or an unusual heart rate, it could indicate autonomic neuropathy. This is caused by damage to the nerves that control functions like the bladder, blood pressure, and sweating.
In Diabetes neuropathy antidepressants are suggested to treat it.

Please report the medications you are taking actually so I can give you further information.

Dr.Mirjeta

I take Lantus and Humalog. I take accuretic 10/12.5 in am (for many years) I take microgestin conrinuously (birth control pill, for many years) but I am off now as I want OBGYN to test for menopause. I take Lyrica 100 mg 2 x day, for many years. Last month the hematologist (no cancer she was following up on Castlemans disease diagnosis from 2009 gave me belladonna to help with sweating. I took only at night but upped to allowable 2 x day. Last Monday the rheumatologist said I may have lower back arthritis and put me on Nabumetone 750 mg 2 x day.
At night i take Ambien 12.5 CR, have for years, and a Xanax, and 1/2 of a XXXXXXX

No meds are new, in fact I've cut my xanax use and was on tylenol 3, stopped it-that was for C5-C6 ACDF surgery in January.

I think it odd that i just have hyperhydrosis out of nowhere? The hands and feet are more sticky than sweaty but are sweaty too, and my feet are freezing.

My bladder leaks occasionally, but only when i get out of a bath.

And at about the same time in late March my hands/feet started the sticky feeling, i cannot sleep well. I was sleeping very well, anout 10 hours, without waking. Now i sleep 4, and was taking two xanax to get to sleep again-i have cut that to one (so i usually take one at bed and one when i wake up, the past couole nights are so bad i take the orher half XXXXXXX to get back to sleep. It seems like then i sleep another 4 but wake several times. but-I'm miserable with not sleeping and the hands and feet and orher sweating. I waited sone time before I went to my MD, and I went in with "Oh i have hyperhydrosis" and i feel like we're on the wrong path.

Lymphoma has been ruled out but my inflammation marker was high. I also am a new celiac-biopsy done 3/30.

Thank you.

Some more info:

Detailed Answer:
Hi again XXXX,
Thanks for the detailed information.
I think the most probable cause of the exessive sweating is of hormonal origin. Especially if it is worsened after stopping the contraceptive pills.
You didn't mention any chronic complication of Diabetes, are you aware of any of them (especially autonomic neuropathy)?
Has your blood sugar been well controlled in general?
You didn't mention any thyroid function control, have you any idea if it is within the normal levels?
Would you attach the result of inflammation markers?
Regards,
Dr.Mirjeta

I have nor been diagnosed with neuropathy. I don't seem to have any complications from diabestes either, I was doagnosed with juvenile diabetes in 1993.
My sugar has been controlled much better as of late, becore getting the celuac diahnosis it was pretty bad. My A1C in Feb 2009 was 8.4, fown to 8.1 in August, then my insurance changed and i was lazy in getting a new endocrinologist, then had to reschedule due to nevk surgery; in March it was 9.8, she ran blood tests (due to weight loss), which led to the biopsy which diagnosed celiac.
My thyroid in February (ran by new PCP) was ok. He looked when i saw him last week.

I will find the inflammation marker test and attach it.

Thank you,
XXXX

The causes of increased inflammation markers:

Detailed Answer:
Hi again,
So the last to exclude are a rheumatological pathology and an infection, as both of them may be associated with excessive sweating and high inflammation markers.

I cannot attach pictures.......the labs out of whack dont seem to be high inflammation. The two not in are 1) under immunofixation, IgM is 259 undercexpected range of 26-217. Under Kappa Lambada free light Chain with ration details Ig Kappa free light chain is 37.01 with expected range of 3.30-19.40 but the hematologist didnt say this. Kappa lambada ratio is 2.04 worh expected range od 0.26-1.65.

Brief Answer:
need to exclude hematological pathologies;

Detailed Answer:
Welcome back,

So, your physician is exploring further for lymphoproliferative diseases and I think, is doing right.
From your description, it does not seem to be caused by Diabetes autonomic neuropathy. The thyroid has been excluded too. So the hematologist would be the next to exclude lymphomas/ amyloidosis ect as a cause of excessive sweating.

Of course, except for protein immunoelectrophoresis and kappa lambda ratio, clinical evaluation and other exams are needed in details.

At this point, I think you should wait for a final response by him.

Kind regards,
Dr.Mirjeta

Brief Answer:
Yes, you should talk to your physician:

Detailed Answer:
Yes, you should ask her about the cause of the high inflammation markers. Of course there is a reason why they are high and why you have symptoms.
There is no cancer, ok. Than an reumatological or infectious disease should be present.

Thabk you, now going back what is your opinion on cause for sweating? Following up-the rheumatologist said perhaps i had arthritis in lower back so said do physical therapy (which I will) and gave Nabumetone.
I have to see a dermatologist today......the whole reason i started this was hands abd feet are sticky, sonetimes sweaty, and other areas too. I saw OBGYN yesterday i am not in menopause.....
Thanks,

As a result;

Detailed Answer:
Hello XXXX,
It is a good news that you are not in menopause!
So, until now are excluded; hormonal disbalance, lymphoma and other lymphoproliferative diseases, autoimune diabetes neuropathy, tuberculosis and other infectious diseases.
The rheumatologist has started AINS for arthritis so let's wait it's effect.
Anyway, if it persists or gets worse I would recommend to see the hematologist again for bone marrow aspiration to rule out non Hodgkins Lymphoma (the only diagnostic tool in some cases).
Meanwhile continue with Xanax and nabumetone and discuss with the gynecologist for the birth control pills, if you need to continue with them or not.
Best wishes,
Dr.Mirjeta
