Suggest Treatment For Insomnia And Headaches

Question: Hi, I was wondering if i could get help interpreting my sleep study results. The diagnosis is moderate OSA with recomended visit to ENT and trial of cpap. In the past few years my insomnia and lack of energy have gotton worse. I have no energy during the day and extreme headaches and attention deffecit. My vision has also rapidly deteriorated within the past year and i now have glasses. I used to be a straight A student and now am about to finish the fourth quarter with 2 Fs a D and a few Cs. I have also become depressed. Everything in life has lost its purpose. My motivation is almost zero. Are these syntoms related to apnea? My AHI is 8.5, AHI nrem 9.3, AHI rem 4.3, I had 47 recorded hypopneas with a record time of 453 min, and a sleep time of 352 min. I am in good physical shape and have a average neck size. When being prepared for the study i noticed the chest band and stomach band were extremely loose which may have flawed data. I also had 3 central apneas. Should this be alarming? thier duration was about 20 seconds each. my sleep latency was 20 while my rem latency was 247. i spent 5.8% in N1, 60.8% in N2, 17.5% in N3, and 15.9% in rem. My arousal index is 10.4: 3.1 apn/hyp arl and 7.3 spontaneous arousals an hour. these events usually caused desats to 93% with 100% being the baseline. XXXXXXX heart rate was 105. If any further information is needed for full diagnosis ask. is Cpap needed? if so what pressure would you recomend? I have exams so my parents want to start cpap treatment ASAP. Finally the last thing im worried about is brain tumor. This could be responsible for insomnia, central apnea, headaches, and rapid deterioration of vision. Is this probable?

Awaiting your response,

Hi
Welcome to Healthcare magic and thanks for putting up the query
From all the reports it is clear that you have Obstructive sleep apnea which means there is some obstruction in the airway which is disturbing sleep due to cessation of respiration.
Please tell me do have any nasal symptoms like frequent nasal infections or throat infections?
What about snoring? Is it there?
Any history of seizures? or any chronic medication?
Follow up with the required information so that I can suggest you better.
Thanksand regards

no nasal symptoms
no snoring
no history of seizures

I had insomnia, attention deficit, and constant exhaustion and headaches so i went to a pediatric psychiatrist who put me on trazadone and adderall, and ordered the sleep study

Also do you think a cpap machine would be the best treatment?

Detailed Answer:
Hi
Your sleep study results say there is decrease in oxygen desaturation of more than 4 % and the Ahi index is also suggestive of mild osas. The apna interval of >20 secs should not be ignored.
My advise is Cpap is the best therapy for you. The sleep study should be repeated with a trial of Cpap to see the results. The pressure in Cpap is variable from patient to patient.
The general body weakness can also be a result of osas as there are frequent arousals in your case.
Cpap has mild side effects that can be easily managed, so I will suggest you to go for it.
Any further questions are always welcome.
Wishing you a speedy recovery.
Thanks and regards
