Suggest Treatment For Insect Bite

Question: my 4 yeaar old son has been developing some bites over a month. The first diagnosis was an insect bite but wast sure what insect extacly, time went by he didnt get better second diagnosis which was last week was scabies, i started the treatment which he is not allergic to Permethrin, as of wednesday of this week he started getting blisters all over his hands , the diagnosis on that was just maybe and irritated scabies part. Yesterday he started developing the blisters all through his body, and this morning he woke up with a very swollen foot( the left one) i waited to see if it would go down and it hasnt. Im very concerned.

I have took him several times to the E.R already and all they send me home with is creams because they feel they are insects bites, within the last month we've been back to the E.R about 6 times. The problem seems to not be going away just getting worser. Is there some test that i can request or that i should request them to have it done. I have never seen a reaction like this with scabies as he was diagnosed. Should i be concerned about the swelling or can i wait until tommorrow morning?
