Suggest Treatment For Inflammatory Bowel Disease

Question: hello,my age is 22.i am suffering from frequent stool problem(daily 3-4 times) for almost 1 year.stool analysis is normal with mucus, veg cell,bacteria present.i can not control my bowel.i am not able to digest any milk or protein mill.No medicine seem to cure it completely.i have also done endoscopy and ultrasonugraphy,colonoscopy.colonoscopy suggests i have proctosigmoiditis.Doctor advised me to take mesacol -od 500 once after diet daily.It seemed to be improved for couple of months but after that it seems to be a chronic problem.Sometimes it feels good and someday it's like previous mucous stool for 2-3 times daily.Otherwise i have no problem.there is not much abdomen cramp but i feel empty stomach within 1-2 hours after diet and can hear bowl sound. Doctor advised me to take orni -o and mesacol-od daily.But it does not seem to improve. i am extremely slim although my father is also slim.but my weight is 44 kg and does not seem to improve nor loss.what should i do?

I have read your queries and will be answered by me.

First,if you are having frequent stools daily 3-4 times that doesn't necessarily mean that you have diarrhea or loose motions.The character and consistency of stool is important rather than frequency.

If your colonoscopy was suggestive of proctosigmoiditis then what you have been prescribed mesacol-OD (mesalazine) is actually an anti-inflammatory drug used to treat inflammation of the digestive tract like in case of inflammatory bowel disease (ulcerative colitis and mild to moderate Crohn's disease).

So,the important thing is to know the underlying cause of your frequent stools,indigestion,dyspepsia whether it is infective or chronic inflammatory.

There could be chances of lactose intolerance also where inability to digest sugar in milk, as stated you.

You must consult gastroenterologist for complete medical history and thoroughly clinical examination with interpretation of your previous medical reports.
You can also go for gut biopsy to have a confirmed diagnosis.

If you have improved with Orni-o and mesacol-od (mesalazine) then your mesacol-OD should be continued till the next follow-up.It takes time (like 4 to 5 months)according to our body response to recover when you are being treated for IBD.

thank you for your valuable advise.actually i am under treatment of a specialist gastroenterologist and have been taking mesacol -od 1.2 gm for 6 months.couple of months ago he had similarly given me ciboz 550 for 1 month alongside mesacol.Then it improved for about 2 months but it seemed to be back from february.I have recently visited him and he has given me orni-o for 20 days.I have taken it for 4 days so far without any further improvement.I like to ask you what kind diet should i follow to get rid of this problem.I do not eat eat spicy food or milky products anymore except curd.my stool is soft and oily,does not contain blood.Stool generally occurs in morning and afternoon.I feel this problem aggravates in summer.I like to know if this is completely curable and how much time does it generally take to cure.I do a lot of physical exercises.I just want to improve my digestion capacity and weight.What else can i do besides taking medicine.If you can give me a diet chart it will be helpfull.

Hi,

Your symptoms might have flares up during the course of treatment.

Since there is no complete cure for Inflammatory Bowel disease,you will have to follow both life style changes and control of flare of symptoms.

I usually recommend my patients to use these drugs like immunomodulators and steroids on flare episodes only.You can ask your gastroenterologist for drug treatment during this flares.

Dietary role:

1) One dietary intervention we may recommend is a low-residue diet which is a very restricted diet that reduces the amount of fiber and other undigested material that pass through your colon and helps to relieve symptoms like diarrhea or abdominal pain or discomfort.

2) If you do go on a low-residue diet, be sure you understand how long you should stay on the diet, because a low-residue diet doesn't let you get all the nutrients you need.You may also require to take vitamin supplements.

3) Avoid foods that may increase stool output such as fresh fruits and vegetables, prunes and caffeinated beverages.

4) Take more omega-3 fatty acids in your diet. which has an anti-inflammatory effect. They are found in fish, including salmon, mackerel, herring and sardines.

5) Decrease or eliminate concentrated sweets in your diet, such as juices, candy and soda, to help decrease amounts of water pulled into your intestine.

6) To take more frequent meals are better tolerated rather than taking full meal at a time. This eating pattern can help increase the amount of nutrition you receive in a day.

Another important thing is to learn how to manage stress, which may worsen your symptoms.

It would be better that you make a list of things that cause you stress and then consider which ones you can eliminate from your daily routine. Also, when you feel stress coming on, it can help to take several deep breaths and release them slowly by blowing out.

Do exercise regularly and spend time with your friends and colleagues.

Engage in activities like sports, reading novels etc.

Try relaxation techniques like Yoga and Meditation.

All these medications,dietary factors,stress management and exercises would have a cumulative effect on your treatment to IBD.

Consult your treating gastroenterologist for further advise.

i have some questions for you.What is proctosigmoiditis?Is it considered to be IBS?I am giving details of my coloscopy report.
INDICATION:Altered bowels
Sedation: Titrated Propofol IV under pulse oximetry
FINDINGS: Multiple erosions seen in the rectum and sigmoid colon. The mucosa from the descending colon to the caecum is normal.
Coming to diet , i do agree that maintaining a low resuduary diet controls stool.But how long i should continue that.its really difficult for me.Just when i try to change it the problem tends to be back again.My doctor has always advised me to maintain normal diet without any restrictions.so if you can give me lists of food that i can maintain in breakfast, lunch and dinner it will be helpfull.Do i have to drink distilled water?Why do i feel discomfort due to stomach rumbling after 2-3 hours of eating.Is it bad? Is mesacol -od is the only medicine of protosigmoiditis and how does it work? If you can please reply to these questions it will be helpfull.

Thanks for the query.
Proctosigmoiditis is actually ulcerative colitis itself(inflammatory bowel disease) but the inflammation is confined to sigmoid colon and rectal mucosa,not involving whole colon means your colon part from caecum to descending colon is normal,also as per your attached reports of colonoscopy.
IBD is a chronic inflammatory disease of colon and can not be cured completely because etiology remains unknown or autoimmune or genetic or in some cases of environmental factors.
There are three major factors who plays vital role to control IBD which are medications, dietary and stress management.
From which dietary and stress management I have already discussed in my previous answers. You must take advise from only gastroenterologist because you will have to take care of your diet specially during active phase of treatment.
Medications like 5 ASA compounds (mesalazine or mesacol or tablet pentasa) with corticosteroids as a strong antiinflammatory drugs,immunomodulators like 6 mercaptopurine,azathioprine,methotrexate used to suppress immune system to decrease the inflammation of colon and to have a quick remission.
It might take treatment for about 4 to 6 months at least with all the three factors mentioned above.
If you will follow only gastroenterologis' advise and take all the three factors seriously then it will start improving within couple of months.
