Suggest Treatment For Inappropriate Sinus Tachycardia

Question: Hello, I have been diagnosed with Inappropriate Sinus Tachycardia in 2009. I was recently switched from Metoprolol to Cardziem because Metoprolol was lowering my blood pressure. On Tuesday I did a 24 hour holter monitor without being on any calcium channel blocker. The conclusion was sinus tachycardia which doesn't suprise me. My only concern and question is regarding the QT of the report. What does this mean and do I need to talk to my cardiologist about having Long QT syndrome? Thank you.

I am Dr. Mody and I would be addressing your concern.
QTc mentioned in the report is for sure on the higher side and needs and evaluation if for sure it's 504 msec. However an important aspect to consider here is its an automatic machine interpretation ( there are high chances that the machine may miss calculate particularly during tachycardia and all around the world no cardiologist would rely just on the same ) and in your case the cardiologist hasn't mention ed anything about the same in his conclusion which is very unlikely if QTc would have been so significantly prolonged. So you may get the Qtc checked manually before arriving to any conclusion.
Lastly do the check the availability of ivabridine in your country of residence , as it's one of the newer drugs for inappropriate sinus tachycardia which no hemodynamic side effects and so you may discuss the same with your doctor .
