Suggest Treatment For Hyponatremia

Question: Hi - I have had several bouts of hyponatremia recently - as soon as reduce my liquid volume, the sodium levels return to normal.
I just had a BMP done last week - I knew I was dehydrated when I went for the blood work.
My creatinine is 1.05 with a normal BUN. My sodium was 140
My GFR was 54.
I'm scared - could my kidneys be failing? I don't have any other symptoms. I have started to increase my fluid intake without any problems

Hi,

water intake of 2litre per day is enough for a normal person. if you try to drink too much water 3 litre or more you will pass more urine even at night and slowly your sodium will come down . now in this situation if your kidney function is slightly borderline then you are at very high risk of developing low sodium and swelling of legs.

some people suggest that drinking water will help your kidneys. this is not true for people with borderline kidney function gfr <60 ml/ min.

but I think you should not panic about your kidneys. keep a check on your blood pressure. and take guidance from your physician

All of this just happened within the last 6 weeks or so.
I stopped both spironolactone and Tegretol

What can I do to improve my overall kidney function?
I took myself to the ER last night because of my concerns.

A CT of kidneys was done - all normal.

Urine normal (no protein), creatinine of 1.15, BUN of 15

No high blood pressure, no diabetes, slightly anemic

Currently, I take NSAIDs regularly, have been on Prilosec for over 35 years
have hypothyroidism well controlled on 100 mcg

I just had a major colectomy done (3 feet of intestines removed) in mid-August - all of my renal problems seemed to start right after this.

No CKD in my family at all!

What can I do to improve my GFR? I'm scared!

I was told that I should follow-up with an endocrinologist to check my pituitary function and levels?

There is no need to panic or fear for kidney failure. serum creatinine is near normal. kidney disease can be due to medicines and pain killers or even unknown factors. in-spite of all investigations exact cause of kidney disease can only be found in less than 50% patients.

So now the the issue is to define whether you have a bad kidney disease which will lead to rapid kidney failure-- it doesn't look like that on the basis of information provided by you. Next we have to understand how to help the kidneys to remain healthy. You have to-

1. don't panic--this will increase blood pressure and central sympathetic nervous system activity and further damage the kidneys.

2. follow doctors advise.

3. you say you have been taking nsaids for long time. Prilosec (omeprazole) is not a NSAID pain killer is there any other Nsaid painkiller you are taking? it may be worth stopping that.

4. avoid non veg specially red meet and decrease your protein intake in diet to less than 0.8 gm/kg/day.

5.avoid tobacco completely.

spironolactone is a diuretic and will cause fall in sodium levels and more urine production. take only in guidance of your doctor

i think your disease will stabilize.
