Question: Hello,
My uncle is a known case of hypertension.he is on telma 40 bd and amlodipine 2.5 bd, presently his kft is derranged, creatinine 2.7, kindly let me know if to change medication for hypertension .

Hi,
At present there is no need to change the prescription. It is important to treat the cause of kidney disease rather than treating kidney itself.
Hypertension alone causing kidney disease is uncommon. We have to figure out 1.whether his kidney disease is due to any other cause, like diabetes or infections or pain killers or allergy or connective tissue diseases or obstructive causes etc;
2. whether it is reversible or not.
3. whether he has any cardiac disease or not
For this, we need any previous creatinine report (any old report that you know); ultrasonography of abdomen to see the size of kidneys, obstruction if any etc; any other drugs the patient is presently or have used heavily in the past like pain killers; 2D ECHO of heart.

he is non diabetic, earlier three creatinine values were between 1.07 to 1.01, usg was done and was absolutely normal, echo is also normal, kindly suggest if any treatment req fr increased creat

Then it is ACUTE KIDNEY INJURY(AKI). Usually it becomes normal in 2-3 months duration. As I told, the treatment of underlying cause usually brings creatinine to normal. The usual underlying causes for AKI are infections, autoimmune diseases, diarrheas and allergies.
He is presently on good drugs. There is no need to change telmisartan as long as serum potassium is normal.
Consultation with a good nephrologist is all that you require.
