Suggest Treatment For Hole Of 3mm In The Septum Separating The Right And Left Ventricles

Question: Hi Dr,

Our first baby boy born on 10th May 2014. He born on 36 week having weight 1.9kg. He is doing good but his doctor recognized an extra sound in his heart on 17th May 2014. According to his suggestion, we have showed to a pediatrician who is in service a branch of XXXXXXX Health Care yesterday. He did some scan and told the baby has a medium hole in his heart (3mm Perimembrenous VSD L-R shunt, gredient 22mmHG) causing the murmur. He told that, this can be closed naturally till 2 years of kids age. He asked us to visit after 2 months. And take care of baby from cold and some precautions. He has given a syrup named some furosemide(not correct) to have 1 dose twice daily. My baby's is having mother milk nicely and gaining proper wait (2.4 kgs now, 28th May).
Can you please let me know the in detail about the cure for this?
Should we use the syrup furosemide?

It is a inter ventricular septal deffect.

Detailed Answer:
Hi,

It is hole of 3mm in the septum separating the right and left ventricles. If there is no hole, blood from the right ventricle go to the lungs, and get oxygenated, and the blood from the left ventricle is supplied to whole of the body. Currently blood from the left is getting shunted to the right. A large hole when neglected leads to increased pressure on the right ventricle and hypertrophy,

Fortunately your baby has a small one. Further weight gain of .5 kg, in 15 days is a good sign of health.

Scientifically it is proved that small holes naturally closes in majority of instances, in a span of about 2 years. If it does not close naturally, he may need to be operated, to close it. But it is rare in well fed children. Any how to have a clear idea of the defect, I advise you to consult a pediatric heart surgeon. He may need to have a echo cardiogram for diagnosis, and confirmation.

Frusemide is a drug used for prevention of the hypertrophy of the right ventricle of the heart.

Except for infections, the defect usually do not cause any problems by its own, if there is infection, it may cause effect on the heart. So any lung infections should be taken care of immediately. As an extra precaution, the baby should be breast fed, and the mother is advised to take green leafy vegetables, pulses, sprouts, and protein rich foods, to give a good nutrition, and resistance to the child.
