Suggest Treatment For High Heart Rate And Anxiety

Question: Hi

In the Year Sep 2013 , due to of anxiety my heart beat gone up by 110, I met with the cardiologist one of the renowned hospital in XXXXXXX . I had explained all my symptom to dr even I used to do smoking in past so dr suggested few test (,T3,T4 & TSH, ECG,2D ECHO , TMT),before reaching any conclusion . TMT found positive in that than dr asked me to go for angiography it’s better to have . I went for Angiography & everything was normal after that Dr told me you stop thinking too much nothing issue in you . But some time once in month or fortnight I feel light pain on left chest where the angiography perform. Soon I did visit couple of visit with my Cardiologist he asked for ECG & everything was normal than he said don’t come to me you are absolutely fine & You need to have counseling as you are suffering from anxiety . Whenever I go for morning walk or morning run I feel that my heart rate is much faster than earlier so I couldn’t run more & some time pin kind of pain on left chest its only for 5-6 sec.

Please suggest with your opinion.

Hi XXXX, I am Dr Mody and I would be addressing your concern.
As I start, some info about yourself, smoking - how much and where did you leave, are you diabetic, obese and what's your family history for the same, also any recreational drug, if at all you take

Now, your doctor did evaluate you with optimum investigation to rule out underlying cardiac condition.
Only thing is the stress test is positive is our concern. However a follow up normal angiogram would rule it as false positive, with only rarer possible of micro vascular disease. However that is not really seen in normal population at your age and only in diabetes and kidney patients

Now coming to the heart rate, remember promod that heart rate is a vital of our body, it means it has to change with body needs. Whenever we work out it should increase, otherwise the heart is not strong, so if it's increasing than rather everything is good, also any mental stress your increase your heart rate would increase proportionately , though 110 is not very high, so be reassured.

Lastly about the recent morning walk,
If you have left doing the regular workout regime, you used to follow previously , again for the same level of activity, you heart rate would be more, which should be how it was previously as soon as you get into the rhythm of your exercise.

Sir in all context of the symptoms you describe, the heart doesn't seem to be a problem. However I would be happy if I can review all your reports including angiogram and any medication you are taking, so I can guide you with a further plan I'd action . even if it's anxiety, it can be easily taken care of. Lastly do you have any spells where you feel the heart is racing off, palpitations

Hello Sir,

I used to have smoke between year 2007- 2011 dec ,was taking 10-12 stick per day, Since year 2012 i stop having somoking . My family doesnt have any history for diabetic & obse , But yes we have have acidity history even i suffer whenver i eat junk food or outside food .Since year 2013 i have faced some personal family stress recenlty (4month back )loss of my mom due to gall blader cancer & even same time some health issue of my spouse put me in pressure .

hello ,
i would start by apologizing for the delay as i wanted to go trough all the reports in detail .
Now i have been to Jupiter hospital ,and their non invasive cardiac department is pretty standard .
the ekg is perfectly normal , stress may be false positive which was ruled out by an angiogram .

So mostly the cause of your palpitation and other symptoms would not be cardiac, i am happy you could wean yourself off from smoking .
now my first impression would be you have a weak stomach and associated reflux gastritis (GERD - gastroesophageal reflux disease , though in milder variants
For this i would suggest
1)avoid heavy meals, small more frequent meals will do the trick
2) loose weight if overweight
3) cut on alcohol, tea coffee, aerated drinks, chocolates for time being
4) no spicy fried food
5) avoid sleeping for atleast 2 hours post meals, dinner by 9 pm.
6) if medication Tab. Sompraz d 1 tab in the morning 15 mins before breakfast for 3 weeks should I help.

Now anxiety would increase the perception than in a normal patient .

In all probabilty , no furthur cardiac work up is needed , still if you want to rule go for any test . thallium perfusion scan , would be the ideal one as you had a stress test before and as it was false positive so repeating the same is not justified , and angiography even CT Scan is too invasive and the XXXXXXX related side effects are not justified , though personally i dont feel the need of it at present

I usually pescribe the following to my patients:
Tab Sompraz D in morning before breakfast
Tab Inderal LA 40 mg once a day
Also i strongly suggest than whenever we are under a stressful condition and we are not able to deal with it , taking some help from a doctor and starting on a anti anxiety drug is not bad , rather it would help you come out of the problem faster and then you can wean it off .
alprax is not the best drug as it has addiction potential and you should take only if sleep is the major issue .
