Suggest Treatment For High Fever

Posted on Fri, 15 Jul 2016
Question: I have had a high(102) temperure that is only controlled by Tylenol 500 mg every six hours.. I have assumed that it is a viral infection but wondering.... This started last Friday night, was very extreme Saturday, but now is only controlled by the Tylenol... I am drinking lots of fluid from water, gator aid ,orange juice, ect...
It takes approx. 40 min to finally break the fever if I waited to take the Tylenol past the six hours... I have not had any since 2:00am this morning... I took a hot shower about 7:30 to try and hold off the fever... it helped some, but my headache and some lower back aches are trying to come back.... we have no insurance so I am concerned to go in.

Detailed Answer:
Hi
I am Dr Mittal.
I have read your message.
I think I can help you.
However, fever has a huge differential diagnosis.
So it is not possible with the limited data given above to tell what exactly you may have.
I will therefore need further information.
Do you have cough?
Do you have breathlessness?
Do you have chest pain?
Do you have palpitations?
Do you have any abdominal symptoms?
Do you have any jaundice?
Do you have loosestools?
Do you have urine burning or any other urinary symptom?
I would request the above information.
Also, have you done any tests?
If yes, upload the reports.
If no, I suggest CBC, Urine routine and culture, Blood culture.
Let me know the information and the reports.
Best of luck, Dr Mittal

No cough
minor breathlessness sometimes with plugged nasal
no chest pains
no palpitations
no real abdominal symptoms... again just telling me I need to eat something
no jaundice
small bowels, due to limited intake... but no real issue
when it first started it woke me up with the urgent need to urinate as it progressed it was still a sense of urgency until about Sunday night... it was very often once every two hours at the longest, but no smell or abnormal colorization to my urine.
I just assumed it was because I was drinking a lot of fluids.... I will say the volume was very minimal until probably middle of the night last night, now more consistent with what I was drinking...
I have had no tests.

Detailed Answer:
hi
i have read your response
there is a definite chance of this being a urine infection
in view of the added information
i think you should go ahead with the tests that I mentioned before

I know we should test, but I have no insurance
I have some Amoxicillin 500 capsule...If I took 1000mg for seven days would this drug even work on a urinary...
I realize that you cannot recommend anything like this without further test, but just curious.

Detailed Answer:
Hi
First, Amox is not a first line antibiotic for urine infection.
Second, even if the antibiotic is taken there may be a partial response. This may result in a subsequent culture to be falsely negative. So it will futher complicate the issue.
Third, as I said before, there are huge number of causes for fever. Its my assumption based on the symptoms that it may be urine infection. We cannot confirm it.
Lastly, the infections nowadays are resistant to many antibiotic. So there is a chance that the bacteria is resistant to Amox. In such a case the antibiotic will not work. the delay in treatment will just delay the whole issue, allowing the infection to spread and thereby further complicating the treatment.
So, in my opinion, I think you should go ahead with the tests, even if you want to start antibiotics thereafter.
Best of luck
