Suggest Treatment For High Blood Pressure

Question: Hi, I have high blood pressure and I've been taking carvedilol twice a day and hydrochlorithiazide once a day. My blood pressure was great a few months ago because I was exercising and eating healthy. Lately I have been just eating on the run. Today my blood pressure was a little high. This afternoon, I had a pop tart and it's been up and down since then. It was 189/106.... Then it finally went down to 135/73 after I rested on the couch. I got up to fix a bowl of oatmeal and it went back up to 149/89. I'm resting on the couch again and it's 152/78 plus I have a headache but it feels like a sinus headache.

Hi, thanks for posting your concern in the HCM.
I think BP of 189/73 is quite high. You are also having fluctuating blood pressure level at different times of the day. I think you are having anxiety related problem. This causes both increased appetite and fluctuating blood pressure.
I need a few information to come to a diagnosis-
1. Did you have chest pain or discomfort or heaviness recently?
2. Did you have fever recently?
3. Do you have any comorbidity other than hypertension, e.g. diabetes, thyroid problem?
4. Do you have any disturbance in sleep?
5. How is your bladder and bowel habit?
6. For how many years you have hypertension?
7. Do you recently have any stress?
However, you may follow the below mentioned recommendations-
1. Salt restricted (<5gm/day), fat restricted diet rich in fiber (oatmeal is definitely good).
2. Continue with your current medications.
3. Continue daily exercise.
4.Have relaxation sessions 30min./day everyday.
You need to have the following investigations done too-
ECG
Blood for fasting TSH, HbA1c, Fasting Lipid profile
Please check your BP everyday at 8am and 8pm and make a BP chart and let me know the same.
