Suggest Treatment For High Blood Pressure

Question: I went to the doctor this morning and was diagnosed with bronchitis. I was given a zpak and an albuterol prescription. My bp at the walk in clinic was 190/90 which scares me. The dr told me to follow up with my dr in a week or so. I am 23 and have had elevated blood pressure all my life but never that high. Is it 1) safe to use nicotine patches instead of smoking (1/2 pack a day) imediately and 2) do i need to see my dr sooner for my blood pressure reading? Thanks

Hi there,

Having a blood pressure of 190/90 at your age is considered high. However, I should know first if the measurement was done immediately after walking to the clinic or after resting for a while.

This is very important as BP can be raised when doing physical activity. The right measurement will require to be at rest for at least 10 to 20 minutes. Then, can measure blood pressure.

Zpak consists of antibiotic Azythromycin, while albuterol is a broncho-dilatator which can give relief of your respiratory problems due to bronchitis.

There are many factors causing high blood pressure at your age:
- heredity
- smoking
- sedentary life
- overweight/obesity

1) It is great that you want to stop smoking. That is the very first step toward final success.

Smoking or absorbing nicotine from patches involves releasing epinephrine (adrenaline) in the body. This is will cause high blood pressure and other related symptoms.

That's why that nicotine patches are not considered safer as compared to smoking. In fact, they have the same effect. You can adjust the dosing according to your weight and former smoking habit. Can talk to your doctor about this.

2) I usually, advise my patients to monitor their blood pressure for at least 1 week, twice daily at home according to the earlier guidelines and record.

You can then, pass the bp records to your doctor who will ask to run further tests:
- ECG
- cardiac echo
- stress test
- lipid profile testing
- diabetes testing
to evaluate your health condition. Depending on the tests results, specific treatment will be prescribed.

I drove to the clinic and sat in the waiting room for a few minutes before my bp was taken. A walk in clinic just means it wasnt my regular dr. Is there a need for imediate concern with my high blood pressure before next week?

Hi back,

It is better to get your blood pressure monitored first. However, if you feel anything unusual (difficulty in breathing, headaches, etc.); should go to ER as soon as possible.

Even in the case when your BP is higher than 150/90 within the first couple of days you start monitoring; I advise to contact your doctor as soon as possible.

Otherwise, you can keep monitoring your bp until next week.
