Suggest Treatment For Hiatus Hernia Other Than Surgery

Question: My mother, aged 66 yrs have been suffering from acidity since long. She has been feeling pain in middle of stomach just below rib cage and in back also. Few days back she observed blood traces in her vomits. On consulting a Gastroenterologist she was advised some tests of blood, urine with UGI and USG.
On investigation Doctor diagnosed Haital Hernia and advised a surgery.
I am concerned whether it is really required or it can be treated with medication, Yoga or exercises only. Please also advise the possible risks associated with this surgery. I am unable to attach the reports here so sending all reports separately through email.

Thanks for your question.
At the outset I would like to say that most Hiatus hernias can be treated with medicines and surgery is not usually required. But, it would depend upon the type of the Hernia and its grade. Also, what level of complications the patient is suffering. If it is a High grade hernia and since you say that she is having complications of blood traces in vomit then surgery might be the right answer. But, I would need to know whether the Doctor saw any gastric erosions or ulcer formation in the stomach or duodenum?
Also, is your mother on continuous medicines like Pantoprazole and Domperidone? If she has had these medicines for over three months without much relief that would also be an indication for surgery.
As for the surgery, it can be done by Laparoscopy as well as open.
The complications are not many and mostly are restricted to a little tightness in the upper abdomen for a few months and might feel a strange feeling while eating food as we tighten the entry of the stomach and bring it down so that almost nothing goes back in the food pipe or oesophagus. So, Surgery need not be feared but should be done if completely indicated.
If I could see the Endoscopy report I would be able to comment with more certainty.

sir there was no other finding like ulcers and my mother was not on any medicine like Pantaprozal. Blood traces were seen only once and might be due to excessive acidity. The white patches seen in the endoscopy were the left overs of last meal(milk) which was taken in night before the test.
After endoscopy and other tests She has been advised some medicines and now she is having significant relief after taking these for one week. All reports and prescription were already uploaded, but I am again uploading for your kind perusal. Please advise whether surgery is really required. I am concerned whether surgery may be avoided.

Thanks for your reply and the reports.
The Endoscopy report does not mention about the details of the hernia.
Also, it does not show any erosions or ulcerations. Therefore, I suggest that she should be put on medicines continuously for 3 months. If she feels relieved then she should continue the medicines life long, though at a decreased dosage.
What are the present medicines she is on. I could advice as to the correctness and dosage.

Hi,
I have reviewed the medicines.
Others can be stopped but Cap Esomac should continue for one to two months. It should be taken empty stomach half an hour before breakfast.
This should help her Gastritis and she should get relieved with this.
She should avoid acidic food stuffs and hot beverages. She should be advised not to lie down immediately after meals and walk a little after meals.
The head end of the bed should be elevated a little to decrease her reflux.
Take care and I believe she would be fine with this line of treatment.
