Question: hi dr my husband suffering from hs purpura.n as the dr suggested us for endoscopy to know about the cause of stomach pain .in endoscopy no ulser is found but hiatus hernia n red sports r found.n in urine report little RBC cell 15-20 n pus cell r found.

Hi,
Thank you for posting your query at HCM.

Read given patient history and went through reports attached to the query.

Following abnormal findings:

1. Hiatal hernia and duodenitis
2. Fatty liver
3. RBC and Pus cell in urine
4. prostate enlargement

Purpura, duodenitis and abnormal urine findings maybe secondary to
Henoch-Schenolein Purpura (HSP) and your advised to visit a dermatologist to be managed accordingly.

Furthermore, lets discuss the findings one by one:

1. Hiatal hernia : hernia is out-pouching of the bowel wall. Hiatal means through the Hiatus ( hole) - in this case the diaphragmatic hiatus. If the patient has NO bothersome symptoms ( heart burn, severe acid reflux, nausea- all daily symptoms) that disturb his quality of life, then observation is advised. However, if symptoms are affecting quality of life , then consult a general surgeon for surgical intervention.

For duodenitis, if epigastric ( burning) pain is experienced by patient OTC Omeprazole or Esomeprazole 40 mg once daily before breakfast maybe ingested for one month after discussing with treating physician

2. Fatty liver may secondary to "increased alcohol intake" and /or increased body weight (fat). Firstly, patient is advised to have a lipid profile and Liver function Test ( blood drawn - for lipid profile should be fasting blood).

Secondly, advice for fatty liver:
- abstinence from "Alcohol"
- LOW fat diet should be followed, AVOID junk food and beverages
- decreased oil consumption (oily food)
- NO red meat
- green vegetables should be ingested daily
- use lemon juice (lemonade) once in a day
- reduce weight if overweight/obese
-"recheck liver enzymes after 6 to 8 weeks" and/or ultrasound.

3. RBC and pus reflects UTI and/or complication of HSP. your advised to visit nephrologist or urologist for further management.

4. Prostate enlargement is mild . It should not be of much concern if NO dribling of urine or urine frequency is experienced. You may consult a urologist for second opinion.
