Suggest Treatment For Hemorrhoidal Bleeding

Question: I am 66 years old male in good general health. I have a hemorrhoid which ends up bleeding once in three weeks on average. I am applying proctozone-hc 2.5 percent once every three days. I am keping away from constipation by taking 1250 to 1500 mg of magnesium daily but the 1500 mg dosage gives me very light gastritis. I am also taking 3 collace 100 mg each daily. My questions are - Is this a good treatment plan, should I use proctozone daily, is it ok to use proctozone daily for years, is it ok to bleed once in three weeks. Thanks.

As per your question answers are-
1)No much more better and safe treatment plans are available these days.Your plan is ok but drugs used are not latest.Docusate sodium and Magnesium are a decade old laxatives and are often not tolerated well by patient's.
Secondly Proctozone should be applied daily before going to sleep locally over hemorrhoids for maximum benefits.
2)Yes it is absolutely ok to use proctozone daily for years,Even some patients use it for decade's.So don't worry about that,Its very safe.
3)Thirdly as hemorrhoid is bleeding once in a month or 3 weeks,Medical management is fine.I don't think you need to go for surgery or other non invasive procedures.Just keep a watch on your hemoglobin levels every month and take additional iron supplements if required.

Now though not asked by you suggested treatment is-
-Stop taking Magnesium(causing dyspepsia) and collace,Instead take Lactulose 10-15ml once daily before going to bed.Lactulose is one of the most frequently used laxative,Often tolerated very well by patients in long terms with minimal side effects.
-Secondly instead of using Proctozone-hc only use a comibination containing contains Calcium Dobesilate, Hydrocortisone, Lignocaine, and Zinc,It will provide much better relief.

-Thirdly ask your gastroenterologist to prescribe you with oral Calcium Dobesilate therapy for 3 months.Initially twice daily for 21 days followed by once daily for 10 weeks.Trials have shown that 3 month therapy can cause shrinkage of Hemorrhoids in almost 60% cases.

-Add Metamucil to your regimen.Its a natural laxative containing Husk.It absorbs water from intestine and swells up making passage of stool easy and add bulk to stool.

-A probiotic should be taken regularly with these medicines.Probiotic improves overall digestion and relieves constipation.

Thank you for your response. It was complete and very helpful.
I take magnesium because it has lot of other benefits for the body besides being a laxative. At 1250 mg and lower dosage, my stomach tolerates it well. Colace is for stool softening. Is lactulose a substitute for colace? How about I take 1000 mg magnesium and little bit of lactulose daily and no colace?
You have suggested using a combination of things rather than Proctozone only. What is the name of this combination. Also when my hemorrhoid has shrunk, should I still apply proctozone or give a one day break.
I had tried one of the latest probiotic and it did not go well with my stomach. Can you suggest a probiotic?

Lactulose is not a substitute for colace(Docusate sodium). It is a laxative much safer and well tolerated by patients as compared to Colace. It is usually prescribed for long term use. Colace (Docusate sodium) is now a decade old laxative and only few doctor's prefer to use it. Latest laxatives are lactulose and Polyethylene glycol.I prefer lactulose in my patients as it has excellent tolerability, However Polyethylene glycol is also a good option.

-Yes 1000mg magnesium can be taken with 10-15 ml of lactulose.
Stop Colace.

-Ruticool or Smuth is trade name of the above mentioned combination.Use it daily as indicated on the leaflet provided with it.
You should not break it,And should use it daily for maximum benefits.

-Along with all these Fybrogel or Metamucil should also be taken once daily.

-Probiotic -Econorm one XXXXXXX thrice daily should be taken regularly.

-Doxium(calcium dobusilate) initially 500mg twice daily for 21 days followed by once daily for 10 weeks,Have also shown good results.

I do not have any itching or pain in my hemorrhoid. Is Ruticool still better than Proctozone, can be used daily for decades, how many years people have used it. I had the understanding that cortisone should not be applied continuously and there should be breaks.
I also have eczema on my lower legs only. Is Ruticool appropriate for that also?

Thanks for follow up.
Proctozone contains Hydrocortisone while Ruticool contains Calcium Dobesilate, Hydrocortisone, Lignocaine, Phenylephrine, and Zinc Oxide.
So both creams contain Hydrocortisone.Ruticool contain some other ingredients which have shown results in shrinking hemorrhoid.So it's superior to Proctozone.
There are two protocols-
It can be used continuosly,However in case severity of symptoms is not that much,as and when required protocol can also be used.
It depends on your convienience.
It can be used till you are ready for any other non invasive procedure or surgery(Kindly don't google too much).

Eczema is a skin disorder and can local application of Ruticool has nothing to do with it.

No ruticool is not for eczema.
