Question: I have recently XXXX. from XXXXXXX to XXXXXXX My son is facing a headache and double vision from one eye. The doctors has done a MRI and told that some veins is swelling in brain hence I approached a neurologist at XXXXXXX who in his opinion that the problem is curable by IV treatment and if not then gone for surgery. Pls. give your opinion urgently about the matter

Thanks for posting in HCM.
I went through all the reports you posted.
All seems normal except the csf pressure which is very high.
But in that condition also his MRI brain is normal.
Kindly can you let me know what injection doctor are going to give him?
Because from the reports there is no reason found for his increased intracranial pressure.
I might give you a better answer if you can tell me what exactly injection your doctor is going to give.
Because MRI should have picked up something for reason for the raised pressure.

Hello, thanks for following up with me.
Headache, vision problem but with normal MRI brain and increased CSF pressure all points towards benign intracranial hypertension also called as idiopathic intracranial hypertension.
I went through the treatment chart and advises and they are exactly according to protocol for these patients. The pressure in CSF is rising without any obvious reasons.
Diuretics which he is already on is the right treatment for this. Also steroid helps in this condition. I couldn't understand the writing but hope he is put on steroids too.
There are other invasive procedures that is removing fluid regularly from back to keep the pressure lower but it has risk of causing infection and other injuries to spinal cord. The surgery is the last option where they will create shunt in csf and other part so as the excess CSF will flow through it and wont cause raised pressure.
But it depends upon the response the patient gives to current medical treatment. If he is responding well then it could be continued without doing any procedure.
But if he doesn't respond to medications then either of the above said procedure might be required.
Also checking his eyes by fundoscopy for features of macular edema should be done to see effect and degree of damage done to eyes.
I would like to tell you that whatever treatment they are doing is the right for this problem. Only ask about addition of steroids to treatment which also will be beneficial. Fundoscopy will give idea of effect on eyes and macula.
