Suggest Treatment For Hair Thinning And Dandruff

Question: hi doctor,
from a past few years I am having hair thinning problems. I use a shampoo and conditioner and wash my hair once a week and if time permits twice a week. After a certain length no matter how carefully I take care of my hair( covering it from dust while riding, oiling it with almond oil olive oil coconut oil mix every week, I do not use hair dryer or straightener which might damage it, XXXXXXX coloured or smoothened it for over 4 yrs and I consume vegetables and fruits as I mostly am a vegetarian) it grows exteremly thin and unhealthy at the ends and does not grow out fast.
I have oily skin, do not suffer much from dandruff, my hair s very frizzy and dry inspite o oiling it well. I took follihair tab for a 15 days but since i got mouth ulcers i was not sure if it was because of the tab so I discontinued it. Kindly suggest a remedy that can improve my dry and thinning hair problems.

I have gone through your query and I have noted down the complaints. Correct me if i am wrong.. You have mentioned that your hair is dry and frizzy, they do not grow as long as they used to grow and they are not thick as before.

I will keep a tentative diagnosis of female pattern androgenetic alopecia.

I would specifically like to know a few more things from you regarding your hair problem.

- Do you also have excessive hair shedding?If yes,how many strands do you lose/day?<50,50-100,>100?

- How often do you comb in a day? Excessive combing can damage hair causing them to develop split ends and become frizzy?

- Are they frizzy more at the ends or throughout? Do you have split ends?

I do not have volume loss or hair thinning in the scalp. i am only referring to hair tips.
yes, I have dry and frizzy hair. they grow out in proper volume.the hair is long and is thick except at the ends.. i.e only the tips are very thin. the hair near the roots are fine, thick and normal.
- Do you also have excessive hair shedding? no, I do not have excessive hair fall it has reduced as compared to before. after a regular hair wash I loose about 100 strands, but when I comb I loose less , about 20.
- How often do you comb in a day? I comb my hair once in the morning.
-Are they frizzy more at the ends or throughout? my hair throughout is little frizzy but the ends are extremely frizzy and rough since it has thinned to a curly frizzy thin end.
Do you have split ends? once the hair become thin in the ends they end up as splits later. i do not now how to prevent it from become thin, rough and unhealthy at the tips to prevent splits and get my hair to grow more.

Hi.

Thank you for the details. It does help a lot.

Your main concern is dry and frizzy hair throughout the length, more at the ends. Hair fall is not much a problem.
You also mention that your hair become thin at the ends and have split ends too.

I would like to keep a diagnosis of Trichorrhexis, specifically "Distal Trichorrhexis".
Trichorrhexis is best regarded as a distinctive response of the hair shaft to injury/ wear and tear.
The most common complaint in trichorrhexis is that the hair is dry, dull and brittle.
The longer the hair the more are the chances that Distal Trichorrhexis is likely to occur.
If the hair is already abnormally fragile, trichorrhexis may follow relatively trivial injury.
However, if the degree and frequency of injury is sufficient, it can be induced in even normal hair.

Traumatic hair care practices that commonly lead to trichorrhexis nodosa are excessive combing, hair straightening/relaxers etc.
Cumulative effect of shampooing, hair dryer use, not protecting while riding etc can also lead to trichorrhexis nodosa.
There is no treatment for trichorrhexis as such, except avoiding traumatic hair care practices and continue to avoid them.

The additional measures which you can follow are...

You can use a wide toothed comb, which will also minimize trauma and should avoid excessive combing.
You can use a gentle cleansing shampoo every alternate days along with a good conditioner to minimize damage.
Scalp itching if present due to underlying causes like dandruff etc if present, also need to be treated.

can you please suggest a shampoo and a conditioner to maintain my hair hence forth ? since u mentioned that shampooing can also cause damage I doubt I will be able to judge which shampoo is mild or harsh for my types of hair.. let me know wat will suit me best.. thank you for ur time!!
