Suggest Treatment For Hair Loss, Dandruff Problem And Sinusitis

Question: Hi i am facing hair loss problem since last two years , mainly in front area. i do have dandruff problem plus sinusitis . sinusitis is very bad. i get sinusitis attacks very often , once in a week , flowing nose, heaviness , tears in eyes . plus i stay in XXXXXXX , water here is very hard . i have started using RO water to wash my hair now . i am also taking tablets from "the vitamin shoppe - for nails,hair and skin which are basically multi vitamins and biotin" Can you suggest something to stop remaining hair loss and strengthen them. one friend of mine visited a doc , she suggested to use anastim lotion, alpecin shampoo and triatop . should i use the same or something else

- Do you see hair on your pillow, bathroom and during combing also.
not on pillow but while combing , yes i do see at times . i hardly use comb these days , use my hand only . also like to mention hair loss is not hereditary but mother do have dandruff prob and weak hair

in summers its less but in winters its like heaps of flakes on scalp, have to scratch at times and see my hair coming out with it
- Is there any itching on the scalp. If yes then is it- mild/moderate/severe.
itching is severe in winters when dandruff is high , its moderate in summers , at times mid also . when i scratch more , more dandruff i see and more hair falls.

- Your dietary status - any substance abuse like alcohol or smoking
diet is north XXXXXXX paranthe's/bread butter with milk(bournvita) , then fruits, dry fruits ,chicken(once in a week) , hardly any junk food , minimum fried stuff , 4-5 glasses of water per day , no smoking at all , whisky once in once week or two (30 ml x 4 drinks) . used to take around 1 azithromicin tab per week during sinusitis attacks(chronic) , in may i suffered from viral fever which lasted for 4 weeks , was on heavy dose of antibiotics then for 4 weeks. have stopped since then. take multi vitamin tabs (vitamin shoppe product - for hair skin and nails)
biting nails habit since childhood

i am in sales profession , so their is stress at times but not consistent as i work from home . do avg 3-4 meetings per week , travel in car . sleep well , 7-8 hours per day .
wake up at 9 , eat breakfast around 10-1030 , start calling or working on computer , travel around 3 times in a week in car in ncr for meetings, come back home , mostly eat home food , hardly any junk food and fried stuff. try to eat as healthy as possible.

Welcome again and thanks for posting your details. After analyzing your query I can make out two basic problems- one is hair-fall or "male pattern baldness" and the other is dandruff or also known as Pityriasis capitis.

As per your query you are suffering from hair fall since long time and worried that the side effects reversibility of hair fall after stoppage of the treatment.

Before I directly come to you concern let me explain you some medical facts regarding hair fall and dandruff.

FOR THE PART OF HAIR FALL: We have almost 1 lac hair on our scalp and amongst them around 5% of hair are in falling phase and then another 0.1% (around 100) of hair can fall everyday. In males this is super-added with hormonal influence of testosterone hormone ( the male hormone) which we call as "male pattern baldness". This also depends upon genetics- meaning type and pattern of hair your family pattern (from your father and mother's side).

This process of falling starts by 20 years and is highest in between the age of 20 to 30 years. I can see you are a young man of 25 years. So it is quite obvious that you can have this problem of hair fall just because of hormonal influence.

Medically the number of normal hair falling varies from few hair to up to 50-100 per day. Now if your hair falling more than 100s per day then it is definitely a problem. If it is not then you have to understand hair fall as normal process of day to day life.

So, as I said earlier your problem is basically Androgenic alopecia or simply saying male pattern baldness. It affects mainly frontal and parietal region of the scalp as in your case too. The temporal and region is rarely affected and occipital region is never affected in this condition. So the most important thing is not to be worried about this normal physiological age related men's problem.

Now regarding the part of treatment:

There are innumerable products in the market claiming to be useful for hair growth. We dermatologists believes in application of Minoxidil lotion. It is one product which is unanimously used by all dermatologists. It is to be started with 2% lotion applied over the scalp once or twice in day. It is to be applied when the scalp is dry. Only 1 ml is sufficient to be applied at the root of hair in frontal region. If for one month there is no burning and irritation then we increase the percentage to 5 % to 10% gradually. The same we do to taper off the medicine. That means we do not stop the medicine at once and gradually taper it off to 5% and then 2% and then alternate day or off and application of lotion before stopping it completely.

For the same you must get personal analysis and accordingly treatment from a dermatologist near you.

FOR DANDRUFF :

Scientifically and medically our skin is a multiple layered structure. Our skin gets renewed every day. The layer which is present on the top today will be fallen off tomorrow. It will not be present tomorrow and would be dead skin in form of scales. This process is not actually visible on normally. So we can say we are just one day old from our skin most superficially.

Now in some persons it is renewed at a faster pace. Because of hygiene and scalp hair this dead skin in form of scales gets entangled and trapped in form of visible of scales known as dandruff. It varies from person to person and also on other of factors which makes the difference inter-individually.

Coming on to your problem which is scientifically known as “pityriasis capitis” is highest in twenties and then recedes by itself in later part of life. It is due to the fact that the renewal of skin and the fatty acid secretion in scalp is highest at this part of life. So there is a tendency to grow fungus as well which makes the flaking more perceptible. There are no such important predisposing factors except for few which can make the situation worsened such as individual's own tendency to renew the skin, application of hair oil and environmental factors.

We can not change our own tendency to renew the skin but the personal measures of hygiene as well as the environmental factors can be modified.

For the remedy part follow the things below:

1.Wash your scalp daily. I guide my patients to wash twice in a single bath. It means when you are washing your scalp apply first time shampoo for washing the hair part only, then rinse it off immediately with water. Reapply the shampoo again immediately, leave it for around 5 minutes, do a good gentle massage by your fingers for around 2 minutes and then wash it off. By doing so for around 7 days you will get good benefit.

2. Stick to one good shampoo which is suiting your hair, don’t change the shampoos frequently. If you are using any conditioner also then to be used over the strands of hair only and not over the scalp skin. There is no difference of shampoos, the only part is to use good company shampoos such as Laureal or Tresemme shampoo. IF the dandruff is too high then you can use any medicated shampoo containing Ketoconazole in it. Therefore it is better to consult a dermatologist who can analyze the exact grade of dandruff and then prescribe medications accordingly.

3.Do not use any kind of hair oil or any oily or creamy stuff any time on your scalp.

4.Comb your hair at least 3-4 times a day and most importantly at night time so that the dust particle and other of dead skin is taken off.So for betterment of high oil secretion the only good solution is daily washing with any good shampoo. Reduce your exposure to hot and humid climate with any consumption of spicy food.


I would like to suggest you to have a good nourishing diet along with all the medicines because that is quite very helpful in retaining the hair and maintaining it better.

Hi

Thanks for the advice, i consulted with a good dermatologist in XXXXXXX . She also said , its mpb
And suggested the med as attached in reports area. Lotion is same as u adviced but she added a tab called as finasteride. She told me in 1% patients , problem of erection is noticed after taking this med ,though it wont happen to you but even if it happens , you can stop it and everything ll be normal again. What do you suggest

After analyzing the situation I am not in favor of finasteride to be given to you. There are few important things in this regard:

- You are young man who might marry nest year. It is one of the basic requirement of every young man to be sexaully active.If even slightest of problem occurs then becomes a big issue for young men like all of us. Though it is said that the erection problem is seen with 1% of men taking finasteride but none of know who can come in that 1%. Finasteride do interfere with sexual potency and therefore even bit of change may cause lot of concerns.

- Finasteride once started has to be given for multiple of months to years of period to get and sustain the results. Considering your marriage next year we can not abruptly start and then stop the drug.

- Lastly there are latest of newer drugs by which the same activity of finasteride can be achieved without its side effects. In topical solution also there is latest of compound with Biochanin-A which is natural phytoestrogen which inhibits both isoforms of 5α-reductase exactly similar to finasteride. There is also saw palmetto extracts which are used to prevent the effect of 5α-reductase. There are no side effects of any of these therapies and have proved to be safest.

So I suggest you to ask your treating doctor regarding your concerns and get guidance for latest of the therapies enumerated above.
