Detailed Answer:
Dear
Thanks for contacting Ayurveda with your health concern and before suggesting treatment options do let me know regarding:

. Is the pathology independent or secondary to some other disease, i.e. constipation is an important cause of the same since when piles develop one is afraid to pass stool with fear of pain & bleeding resulting in constipation and due to constipation he has to strain hard to pass stool resulting in piles and cycle continues.

. any opinion of general surgeon taken?
. your life style: sedentary lifestyle (sitting on hard floor, ) or active?
. diet: do you take enough fibre in diet and drink plenty of water?
. do you suffer from digestive problem?? (since in Ayurveda we hold that people suffering from digestion problems do suffer from piles)
. habit of taking spicy food?
. any family history of the same?

     It started in last October, one day when I had a constipation and I forced a lot (which could have been avoided) and it started bleeding. After that I had to travel outside XXXXXXX due to official work and couldn’t start the treatment. I don’t have a constipation problem nowadays but the rectum doesn’t open during passing stool and it feels like paining and pinching from inside. Also, there is a small grown piece just outside the rectum.
o     any opinion of general surgeon taken?
     I consulted a homeopath doctor after returning in Nov 2015
o     your life style: sedentary lifestyle (sitting on hard floor, ) or active?
     I’m an IT professional so have to sit on a chair for a long time. However, I’m an active guy and do regular exercise (including 4-5 KM running and weight training)
o     diet: do you take enough fibre in diet and drink plenty of water?
     I drint enough water approximately 4 liter in a day. Not too much but my daily food includes lots of protein, and vegetables.
o     do you suffer from digestive problem?? (since in Ayurveda we hold that people suffering from digestion problems do suffer from piles)
     not in general but once or twice in a year
o     habit of taking spicy food?
     I like spices but not hot. Also, I don’t take too much spicy food on a regular basis.
o     any family history of the same?
     My father has the same problem but it’s not severe. He doesn’t need to consult a doctor for the same.

Brief Answer:
Ayurveda management

Detailed Answer:
Dear
Thanks for reverting back.

A. In best interest it is wise to seek an opinion/examination with a general surgeon from modern side or shalya doctor from Ayurveda stream since treatment of fissure in both pathies is surgical correction of the same if not managed with medicine.

B. It is easier to ligate and remove a proturding/bleeding pile mass but this only gives temporary relief, so if the basic cause of piles such as constipation and sluggishness of liver is managed then risk of secondary infection (with surgical interference) is greatly reduced.

C. I will provide with the dietary & treatment guidelines which is suitable in both the diseases and in the mean time just have an opinion of general Ayurveda surgeon.

D. Take: old brown rice, barley, horse gram, green gram, bottle gourd, turnip, green vegetables, papaya, apple, cucumber, XXXXXXX butter milk, sugar candy, elephant yam, dry fruits, pears, raw radish, brinjal, carrot, drumstick, capsicum, tomato, beetroot, broccoli, raisins, prunes

E. Hot sitz bath in a tub of warm water for 10 minutes (2 times a day), use XXXXXXX latrine (squatting) instead of western (commode)

F. Yogic exercises like: Padmasana, vajrasana, halasana, sarpasana, XXXXXXX prakshalana should be learnt under the supervision of experts on yoga.

G. 1 glass lukewarm water early morning, night
