Posted on Thu, 27 Apr 2017
Question: Good evening. I am a 45 year old man who is obese but otherwise in good health. I have a pilonidal dimple and in the past 24-48 hours I have experience mild to moderate burning/pain in that area. I also believe I have mild hemorrhiods. When I inspect the pilonidal dimple area I don't appear to have a cyst, bumps, blisters or but the skin is a little red. Other than that I don't have any other symptoms? Any thoughts as to what this might be?

Brief Answer:
Self care & needful expert advise would help.

Detailed Answer:
Hi John_c_sweeny,
Thanks for the query. I have gone through it & understood your concern.
The pain & burning& red skin you experienced since last two days in the pilonidal dimple- without any cyst, bump seems to be out of-
Sweating leading to probable itching & inflammation of the area.Possible fungal/ bacterial infection can not be neglected. It does not seem to be related to haemorrhoides.
* Home care - If there is no other symptom ( as you had already stated)- it needs just cleaning & drying of the part & application of some antibiotic& antifungal dusting powder or ointment( Neosporine ) .
- Excessive sweating making the part damp & warm should be avoided by using cotton or hosiery under garments ( which can soak sweat)./ frequent cleaning.
-A non steroidal anti inflammatory pill like Ibuprofen ( 400mg ) over the counter can be taken twice a day after food. add with Ranitidin 150 mg to be added if you have hyper acidity tendency.
- If you are a diabetic ,blood sugar should be checked . It should be well controlled/ Treating doctor's advise to be taken as per need.
* Doctor's advise- In case it does not resolve within 5 days You have to consult a physician for necessary oral antibiotics.
* The suspected haemorrhiodes -can be self controlled by avoiding constipation... for that,
- Eat ample of fibers in diet ( veges/ salads/ fruits with skins )
- Drink at least 10 -12 glasses of water in 24 hours.
- Have a big tumbler of warm water with small spoon of honey & !/$th lemon juice- first thing in morning.
- At least 44 minutes regular exercise( walking, yoga exercises ) daily would help
- Consult a surgeon in case it bleeds.
I hope that answers your query. Any follow up query is welcome.
Thanks.
