Suggest Treatment For Gestational Diabetes

Question: Hello Doctor,

Fortunately, after trying numerous times my wife did again conceive and the recent scan showed 9 weeks pregnant with fetal cardiac activity recognized. She was as ussual on Thyronorm 25 mcg. Post she conceived and looking at her history of missed abortion, our local Gynaecologist prescribed her the below.

1. Duphaston 10 mg twice a day.
2. Ecosprin 75 mg once a day.
3. Folic 123 once a day.
4. Fertigyn HO 5000 LU (Once a week injection)
5. Progeterone vaginal tablet, once a day at bedtime.

Her fasting glucose is normal but PP was at 160. As such, our doctor advised us to consult a endocrinologist for monitoring the sugar level.

As such, we consultate an endocrinogist whi had prescribed the test as below. I am also highlighting the results.

1. Fasting : 93 mg/dl
2. PP : 158 mg/dl
3. HbA1c : HbA1c was at 6.7 % and eAG was at 145.6 mg/dl

Post the testing, the doc immediately prescribed her NOVOMIX 50 Hexpen with 6U SC everyday throughout her pregnancy, due to her history of miscarriage and a family history of diabetes(father), as the results tends to Gestational Diabetes.

My wife is absolutely in panic mood. AS the same was prescribed on 12th Feb 2015, we have still not had a dose.

Just thought to have your views as well on this pls. I have attached the recent blood test report and both the prescription of the gynaecologist & endocrinologist.

I understand your concerns about diabetes during pregnancy. This is called gestational diabetes mellitus (GDM).

Your wife does not have such high levels of sugars to warrant insulin.

I don't think diabetes is the reason for miscarriage earlier.

Miscarriage earlier is likely to be due to chromosomal problems.

Please take second opinion from another doctor before going ahead with insulin.

Let me know which city you stay so that I can recommend a specialist if you wish.

Diet modification is enough at present to control sugar levels.

Dear Doc,

Appreciate your advice.

Due to anxiety, she had taken the insulin as recommended by the local doc. 6 units Novomix Hexpen. Yesterday night she had severe bleeding at around 1030 in the evening and was immediately hospitalized.

USG scans revealed the below impression.

"Early intra uterine pregnancy at 10 weeks 4 days of gestational age. Sub Chorionic and sub membranous hemorrhage collections with clots. Placenta is fundo body anterior.

Grade I maturity. Not low lying.
Small amount of anechoic fluid collections is noted in fundus and sub-chorionic region in anterior part of sac.
Large fluid collections of approx 22 ml is noted inferior to sac just above the internal os. Echogenic area suggestive of clots are noted."

Kindly advise. We are from XXXXXXX

Majority of women with such bleeding continue pregnancy successfully and deliver a healthy baby.

Just follow doctor's advice and continue medicines. She will be fine.

Let her keep track of sugar levels since she has already started insulin. Keep reporting sugar levels to doctor.
