Suggest Treatment For Fungal Infection On The Lower Back

Question: Hi.. I am XXXXXXX I am 25 yrs old weighing around 100 kgs. I seem to have a skin problem. My face was initially white in color without any problems till my 16 yrs of age. But after that suddenly probably because of sun exposure i guess, my face and neck started becoming black. So we went to a skin doctor and there was some bad reaction from that doctor and suddenly i started getting pimples and slowly the pimples turned into black spots and seem to cover whole face. From then on i was regularly going to doctors and did not see any results. The main problem is that I get irritation on my face (specifically on both sides between eyes and lips, where my face is very black). In some cases when I used to write ointments, the skin even used to peel mainly from that face position and causes irritation (even now). From around ten years I must have got to at least 10 doctors, but to no avail. Recently I had a fungal infection on my lower back. So I went to doctor and at the same time I asked about the face problem too. He mentioned one of the main problems for both is my over weight. At the same time he gave me a facewash, sun protect cream and couple of other ointments for face. And it has actually become worse. Not sure how to handle this issue. Please help.
I am attaching the recent prescription of the doctor along with my face photos and the face wash the doctor prescribed.
FYI: My whole body is white, only the face and neck part is black.

Hi Doctor... The last Doctor that I visited asked me to use 2 ointments Dermadew glow and Dermadew lotion in the night and morning respectively. He also asked me to use ahaglow face wash. But still the irritation persists. So please guide me. Thanks

Hi. I have reviewed the Images

Yes, this is acanthosis nigricans and it is related to insulin resistance commonly seen in overweight individuals. Acanthosis manifests as dark velvetty skin mainly in body folds(neck) and also on face and extensor surfaces.
I suggest you to start on Tab. Canthex(contains liopic acid), twice daily(morning and evening) for 3-4 months. This works by countering insulin resistance which is the main cause of acnthosis nigricans in overweight individuals.
First of all you should try to reduce weight as this would also help in addition to treatment. You can discontinue all topical treatment as this is causing irritation.

Hi Doctor. Thanks a lot for your diagnosis. I have some more clarifications. 1. Right now should I use only Canthex for countering Acanthosis along with some steps to reduce weight or do I have to do anything else in this 3 months time 2. Should I follow any specific diet while using these capsules (FYI I am a vegetarian) 3. Are there any side effects of using Canthex capsules when I should see it as a time to revise medication 4. Generally the affected parts of my face are very dry and every time I am washing my face it seems to irritate a bit. Can you let me know any face wash I can use along with a soap (I generally use only face wash to face and don't use soap, pls let me know if that is fine. The face wash that I am using is Ahaglow and that do not seem to work. Please let me know if I continue the same or change) 5. After starting using the capsules after how long can we see some results and when should I contact you again for a interim check.
Finally is Acanthosis completely curable and how long will it take for curing it completely. Thanks a lot for your patience. Appreciate your detailed answer.

Yes I suggest you to use canthex capsules only. There are no side effects of canthex capsules.
It may take upto 3-4 months for the results to show.
Irritation that you experience on face is due to the face wash. I suggest you to stop using it. You may rather use gentle cleanser e.g cetaphil cleansing lotion.
