Suggest Treatment For Frequent Urination Along With Blood In Semen And Urine

Question: I'm a 49 yr old male hat went to urologist with frequent urination and blood in my semen and urine. That began a series of tests with the following findings. Urologist performed a cystoscopy and remove a blood clot from my prostate. Did not find any signs of bladder cancer. My PSA is .7

CT and MRI of Pelvis w/wo contrast.

Mixed density lesions in pelvis on CT may represent a combination of enchondromas and hemangiolipomas and a few scattered more typical boney islands.

There is a small to moderate amount of fluid in the posterior right pelvis. There is some peristaltic change seen in the adjacent sigmoid rectal colon w/o otherwise mass for technique. The prostate has some subtle increased signal at the posterior left aspect on the T2 weighted sequences with some subtle associated enhancement post-contrast. It is slightly indenting the bladder base.

There is a prominent lobular residual thy mic tissue at the anterior mediastinum, particularly for the patients age. There are a few shorty axillary lymph nodes.

There is a large paraesophageal hiatal hernia (prior Nissen Fundoplication in 1987)

I am scheduled for a bone scan in my pelvis.

Based on the info above, do you think I could have Chondrosarcoma and/or Maffucci's Syndrome?

Hi

I understand your concerns. But the description on CT appears to be benign lesions. There is no suggestion of chondrosarcoma here. Mafucci syndrome patients do have multiple enchondroma with hemangiomas but they are more in the extremities. Patients present much early in life. So I will be extremely surprised if this turns out to be Mafucci.

Even for an adult, a diffusely enlarged thymus does not need much attention. Most commonly it is benign. Only if there is a discrete mass, it needs further evaluation.

Let's see what is the final diagnosis, but I feel it will be benign.

Thank you for your reply. Whst do you think may be the reason/cause of the enchondromas and hemangiolipomas, along eith the fluid in my pelvis? Could these be related to my prior bleeding prostate? There are also what appears to be benign cysts on my liver. Should all of these things be connected or viewed separately? I appreciate your time .

No these are unrelated

enchondromas and hemangiolipomas are usually developemental anomalies, may be congenital or occurs during growth. these are benign. Not related to bleeding from prostate in anyway.

Benign cysts in liver are nothing to worry of. They are unrelated to bone or prostate problems.

What about the fluid present in the pelvic area as well? I have had imaging of my pelvis at some point over the last 10 years and it wasnt there .

may be due to infection

I am not sure how much fluid is there. Small amount is normal. It may be due to infection in the prostate also.

yes some fluid is there

Detailed Answer:
This fluid is unusual but it is not related to the bony lesions. Those are definitely benign. It may be related to some prostatic infection which can explain everything.
