Question: Dear Sir/Madam,

I am writing about a situation of my sister. She is 33 yrs of age.
Once she fell sick of typhoid in 2007 and after that she has been suffering from flatulence problem.
Her stomach bloats after eating and she belches a lot (at least 10 times) after having food in lunch and dinner. When she belches it seems as if a flow of air is coming out of her mouth.
She goes to toilet at least 5 times a day for eliminations. I think that she suffers from constipation as well. She goes to toilet just after having food also so that she could get rid of gas in her stomach.
She has been taking Omeprazole and Rebeprazole capsules and digine tablets to get relief from her gas problem but that fail to control her problem on complete basis. After relief of few days her belching problem returns.
She has been a patient of anxiety neurosis as well. She used to suffer from heart palpitations and for that she was given Inderal 10 and Melzap MD .25 for a day. She had had Melzap MD .25 for some 5 years.
Now she is fine but I am writing her anxiety history so that it gives you a complete knowledge about my sister’s problem and so that it helps drug suggestion by you.
She feels really troubled by her situation related with gas, please help her.

I have gone through the details given by you about your sister.
I can understand your concerns.

Dear Mr. XXXXXXX
She has complaints of belching, flatulence, bloating and frequent visits to washroom (probably due to constipation).
She has problem of anxiety also.

Such gastric complaints in background of anxiety and typhoid are mostly suggestive of irritable bowel disease (IBD) or malabsorption syndrome.

I think your system has constipation predominant IBD. In such cases, drugs like levosulpride (prescription drug) are effective.

She should take esomeprazole instead of rabeprazole as it is a long acting antacid drug.

Other than this, darolac-IBS capsules, which are lactobaccillus preparations, can also help.

B-complex vitamin supplements are also essential as there may be deficiency due to malabsorption.

Also, relaxation exercises like deep breathing and meditation should be a part of her daily routine.
Drinking plenty of water will also help as it reduces oxidative stress in body.

Thanks for your reply.

You are right sir,whenever she takes Vitamin B-Complex syrup or Tablets she says that she feels better.

Anyways, sir, could you please tell me any brand name for esomeprazole and how many times this esomeprazole and Darolac -IBS capsules have to be takes.

Especially, whether Esomeprazole have be taken empty stomach and whether Darolac IBS have taken after food? Please specify sir.

Dear Mr. XXXXXXX

Esomeprazole is available under brand name ESOMAC (by Cipla) or ESOZ (Glenmark). It has to be taken empty stomach once a day in morning. It is available in 20mg and 40mg strengths. She take the dose with which she feels well after consulting the doctor.

Darolac-IBS can be taken once a day. It can be taken before or after food, anytime.

Kindly discuss this with her doctor.
