Suggest Treatment For Finger Numbness And Elbow Pain

Question: Hi there. I am a very active female, 47 years old physiotherapist. I have recently being experiencing some random symptoms which are worrying me a lot at the moment. It started about 10 days ago with a burning feeling in the left ring finger. I have a fused DIP joint from a previous injury so have mild osteoarthritis in that joint. After 3-4 days I started to notice a burning, tingly pain in my right hand thumb and first two fingers and in the palm as well, one night it was particularly bad and felt like my fingertips were burning. I went to my GP who diagnosed me with carpal tunnel and have worn a wrist splint for the past two nights. But at the same time I have had some pain in my right elbow it doesn't feel like joint more an ache and just now I played tennis for an hour and a half and got numb pain in my right foot under the 2nd, 3rd and 4th metatarsals. Any ideas of what is happening...I am concerned and having trouble sleeping at night.... I am well taking only the occasional bit of Ventolin for exercise induced asthma. I have had no virus and a general blood test 3 months ago was clear.
My history: Take no medications except for Ventolin 1-2 x a week
Had right ankle operation on to repair right peroneal tendon 2012 - nil problems
Had 8 surgeries left ring finger due to severing FDP and FDL with palmaris longus tendon. Infected from a stitch and operated on March 2015 had strep in it but nil problems since.
Had tonsils and adenoids out and 3 x sinus operations last one 2011
Still have regular periods no signs of stopping no menopausal symptoms
Get some Raynauds in fingers nil medication feet are fine
Non smoker, non drinker and exercise 5-6 hours per week running and playing tennis
170 cm tall and 57kg weight
Still have the numbness in my three fingers like carpal tunnel but some lateral epicondyle type pain in both elbows but surely I can't have bilateral tennis elbow and right sided carpal tunnel. Really worried...would appreciate an answer as soon as you can....

Uric acid level should be checked.

Thank you for asking HCM.

I have gone through your query. High uric acid level also can give such symptoms like burning sensations and pain especially at night. So better go for a blood test for uric acid level. Vitamin D and B12 should be checked also.

How is your ESR (erythrocyte sedimentation rate) levels. ? If it is increased a systemic rheumatologic condition can be suspected. In such case a consultation with rheumatologist and ANA profile as well as RA factor tests should be done also.

If all these are fine then a nerve conduction study should be conducted to rule out any entrapment neuropathies.

Thanks for the answer. I will contact my GP in the morning. I have just woken up at 2am in morning with burning pain in my right toes and sweating. There is no sign of redness or inflammation. Does that add anything to the equation? Appreciate your help.

Hi Dr. XXXXXXX

I really appreciate your advice. Rang my GP this morning and he ordered a full range of blood tests for screening however won't know the results for another 72 hours. In the meantime finger numbness seems to be centred on a median nerve type format however with less burning and the elbows right in particular seems to feel as if I have leaned on something a bit sore.

I also have been getting the numbness bilaterally today of both feet medially and under 1st to 2nd metartasals and big toe.

My question which you probably won't be able to answer is could it be a systematic disease like fibromyalgia or multiple sclerosis. I guess as I work as a physical therapist a little knowledge is dangerous.

I also have some prednisone and amitriptyline from past neck issues and asthma flare up. Would it be worth using one of those.

Do you think contacting a neurologist online would be worthwhile as well or should I wait until I get a clearer picture? Appreciate your time. XXXXXXX
I meant to say the numbness to my feet comes on when I drive particularly...

Diagnosis of fibromyalgia is put when your tests and criteria for other problems are within normal limits. In fibromyalgia there is also sleep disturbances. It usually improve when you get a good sleep. No need to think about MS. Be relaxed and wait for the results.

You can take over the counter NSAIDs like Ibuprofen , have a hot shower and get a proper sleep also. Online neurologist consultation helps only for advise. Normal consultation is required to have full neurological examination which is the main basis of neurology consultation.
