Question: I am a 50 year old female who has had a history of fibroids and ovarian cysts. I did a procedure of fibroid embolization. I was unsuccessful with that procedure I was told that the fibroids had shrunk. But recently I had a very terrible period which I was bleeding heavily. Its an 8 day cycle. I took a test for cervical cancer and it was benign. It was suggested to get on a birth control regiment but Im just figured out what is the real issue

Brief Answer:
You need a more thorough evaluation

Detailed Answer:
Thank you for the question. I have the following questions for you:

1. Have you had a biopsy from the inside of the uterus (endometrial biopsy)?
2. For what reason was hysterectomy not considered as an option?
3. Do you have other medical problems??

No biopsy inside the uterus just the cervix. I opted not to have the hysterectomy because I thought I was approaching menopause and that my cycle would taper off unfortunately it hasent I still have heavy bleeding during my cycle. Im not bleeding any other time. Yes I do have a history of hypertension and I have fibromyalgia

Brief Answer:
You need evaluation of the uterine lining

Detailed Answer:
Thank you for the follow up information and I am sorry about the delay.

I understand the reasoning behind trying to avoid surgery and the reasoning behind the use of uterine artery embolization made sense. Any time a woman has abnormal bleeding over the age of 40, a sample should be obtained from inside the uterus to evaluate for pre-cancerous changes. I am surprised that this was not performed and this should be the first step. If they are unable to get a sample from inside the uterus, then you may be faced with a hysterectomy.

If they are able to get a sample and there is no evidence of pre-cancerous changes, then it would be reasonable to make attempts to control your bleeding using some version of hormones to decrease your bleeding.

My gut feeling, though, is that the most effective way to deal with your bleeding and make sure that there is no concern with regard to your uterus is to go ahead with a hysterectomy.

Please consider getting back in with an OB/GYN so that this can be addressed. Please also ask if you have any additional questions.
