Suggest Treatment For Fibroid In The Posterior Wall Of Uterus

Question: I'm 32 years old, no kids, 5'7, 115lbs with a Fibroid inside the posterior wall of my uterus measuring 2.12cm. I'm having frequent urination. I was told by my doctor that this is not the cause of my frequency and he scheduled a follow up US in 6 months. I had a cystoscopy, ct urogram, renal ultrasound, blood work including stds and everything was normal. I take diflucan once in a while for candidiasis, I had it since I was a teenager and honestly it never get cures. I don't know what to do or where to go, I keep on having frequent urination and sometimes a pressure like an urge to pee, I wake up almost 3 times at night to pee. I appreciate your advise Doctor. I am Dr Madhuri a consultant gynecologist and will be helping you in your query.

The fibroid that you have is small and is located in the posterior wall that is on the back side of the uterus. The urinary bladder lies in front of the uterus. So it is unlikely that the fibroid is producing these symptoms. Fibroids of this size are very common and incidentally found in women on ultrasound. Most of them do not need any treatment.

Now about the frequent urination: These symptoms may be produced by a number of causes. The commonest cause being urinary infection and diabetes [ raised blood sugar]. Hyperhydrosis is a condition where there is frequent urination due to involuntary intake of large amounts of fluid.
Other reasons are an over active bladder, stones, and urethral incontinence.
Nerve damage to the bladder also may cause this. Pelvic floor muscle weakness also causes such symptoms and conditions like multiple sclerosis may also affect the bladder.

So the best way is to get a urine culture test as well as a blood sugar test done. If these are normal then investigations for an overactive bladder like cystometry will be needed after discussion with a urologist. Also check if you are taking any drugs that can more urine like diuretics.

Meanwhile try to take as much fluid as possible till 6 PM and then try to restrict it so that if it is hyperhydrosis you will not have the symptoms at night and the diagnosis will be established.

Exercises to strenghthen the pelvic floor muscles [ kiegels exercises] need to be started. They are simple and information is available online. So do them as many times as possible during the day. The effect takes 2-3 months to show results when the muscles begin to gain strength.

I tend to prescribe an empirical course of antibiotics as sometimes the bacteria are not detected in urine [ example is interstitial cystitis]. A course of a good urinary antibiotic like nitrofurantoin helps in may cases and can be taken after prescription. If it is an overactive bladder drugs like oxybutinin can be taken after prescription to help decrease the symptoms.

With this I think that most of the causes will be ruled out. So kindly discuss these with your doctor and start the exercises as soon as possible.

Thank you Doctor for your prompt response. I’m very happy with your service and explanation. I had a cystoscopy done to rule out intersticial cystitis, everything came normal as well for the CT Scan Urogram I believe, renal ultrasound normal, and blood work and urine culture taken directly from inside the bladder using a catheter, I don’t remember the name of the procedure. I will definitely discuss overactive bladder possible cause with my doctor. Thank you very much. Have a great day
