Suggest Treatment For Fever, Sluggish Behavior And Diarrhea

Question: I have question about 3 years old child. We were out yesyerday on hot sun.. he was in stroller and covered plus fan over him. In subway was really hot and we had to sit in train 30 min. He was fine last night and ate his food but starting this morning he has fever.. first fever was 101, second 102 and right now 101.5. He is sluggish, does not want to drink or eat. he did drink about 300 ml all day. I have been giving him children Tylenol. He also has diarrhea and wants to sleep all the time. I also noticed faster breathing and usually. In this point my question is is it possibly heat stroke, heat exhaustion or does it seems like something else. also what would be smart thing to do in this point.

By what you quote I feel it could be a viral illness or a heat exhaustion.

Fever of few days without any localizing signs could as well a viral illness.

Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require anything except Tylenol in appropriate dose and hydration. Even if it's heat exhaustion keep him well hydrated and make sure he passes good amount of urine at least 6th hourly once.

Regarding diarrhea, this again points out to viral illness. If diarrhea is severe then you need to give him ORS and Zinc supplements.

Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.

Paracetamol can be given in the dose of 15mg/kg/dose (maximum ceiling dose of 500mg) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.
