Suggest Treatment For Fever, High BP And Profuse Sweating

Question: My husband was very ill one night. He was sweating profusely and couldn't drive home after having a drink with his friends. He rested at home. He was throwing up, and felt like a beer had poisoned him. He had fever and chills. He was very tired. On the following morning after the night it happened, we had unprotected sex. 3 days later, on Friday, I had issues with my blood pressure shooting up high. Then it happened at work every day for three days. Prior to that, I had been experiencing stress and moving books from my classroom, moving to a new apartment, driving back and forth between cities to get to work, and not eating healthy. All of that happened over a month ago. Now I am experiencing daily shortness of breath and chest pain all day. Ekg is fine, lung xray is fine. I have no appetite and no energy. If my husband were poisoned and ill, do you think that his semen could have placed a bacteria or toxin in my body or even a poison in my body? (I was able to stabalize my blood pressure through diet and it is now lower. I also keep a log and it is low daily). What is the possibility of a poisonous chemical being transferred from his body to mine via semen?

Brief Answer:
Your symptoms seem to be related to psychological stress

Detailed Answer:
Hello!

Thank you for asking on HCM!

Regarding your concern, your symptoms seem to be related to psychological and physical stress.

I would like to explain that a gastrointestinal infection ( including food intoxication ) has really low chances (almost 0%) to be transmitted through sexual ways.

A sexual intercourse can cause sexually transmitted diseases but not a gastrointestinal infection.
From the other hand this clinical scenario doesn't seem to be related to any infection.

The fact that your blood test, ECG and chest x ray have resulted normal is very reassuring that nothing serious is happening with you.

I recommend monitoring your blood pressure regularly and avoid psychological and physical stress for the next days.

Hope to have been helpful !

I am at your disposal for further questions!

Greetings! Dr. Iliri

If I were to have gotten ataxic or Biot's breathing due to mistakenly using Ashwaghandha (and not knowing it could cause respiratory issues) what treatment would work for me? I know that cpap was not effective and that there is not much treatment out there. Please tell me what possible treatment is out there. (new meds)

Brief Answer:
You need to ask medical attention to a specialized toxicology center.

Detailed Answer:
Hi again!

Ashwaghandha is a herbal supplement with very extensive pharmacological effects, especially on central nervous system (it acts by inhibiting acetyl cholinesterase, affecting so cholinergic neurotransmission; also dopaminergic, adrenergic, serotoninergic gabaergic transimssions as well.

So, using such a compound (though it may have several beneficial effects), could yield numerous side effects (especially when an overdose comes to scene).

At that point (centrally mediated respiratory complications), i would recommend to immediately seek specialized medical assistance in a toxicology center, as the treatment remains symptomatology related.

A comprehensive evaluation should be made, including special blood level measurements of Ashwaghandha compounds concentration, and closely monitoring vital parameters.

Facing respective symptomatology and lab results, the medical staff would decide the right therapy (cholinergic syndrome, or serotoninergic, dopaminergic, etc).

I strongly recommend to run to a Toxicology center for further evaluations.

I discourage you from continuing on such a therapy with so many unpredictable side effects.

You need to discuss with a neurologist/psychiatrist for a safer therapeutic alternative (respective to your primary health disorders).

Best regards,

Dr. Iliri

If the damage turns out to be irreversible and I have to seek a neurologist for problems with a weak central nervous system, would he be able to provide me with any form of medication to stimulate my cns? Do those types of meds exist? My chest, shoulders, and stomach are very weak. I notice that my Paxil is helping me to breath better. Also, I know that since Paxil is an Serotonin uptake inhibitor and will eventually deplete my serotonin levels. My breathing issue will return at that time, unfortunately. What is the name of therapy that exists for central fatigue syndrome or cns related fatigue?

Brief Answer:
Many types of antidepressants are effective in chronic fatigue syndrome

Detailed Answer:
Hello!

I understand your concern and I would like to explain that Paxil acts by increasing serotonine levels in your brain cells by modifying in a permanent way the cell receptors.

So if continued for a proper period of time and if stopped slowly, its interruption doesn't usually cause relapses of the symptoms.

There are a lot of other antidepressants (like doluxetine, venlafaxine, sertraline, etc), which can be used in chronic fatigue syndrome if this therapy shows to be ineffective.
There is not a specific therapy.
They all act by increasing the brain levels of different neurotransmitters (like serotonin, acetilcholine, dopamine or adrenaline and noradrenaline) causing stimulation of the cells in different regions of the brain, which are responsible for this clinical scenario.

As long as Paxil is showing good effects on you, I recommend continuing it. This treatment may be needed to go on for months.
I advise consulting with your attending physician on the above issues.

I would also advise a healthy diet which is essential for a healthy mind and avoiding alcohol and caffeine.

Hope to have been of help!

Best regards!

If a person experiences peripheral nerve damage or a slowed cns due to a drug or vitamin what is the prognosis or outlook for the individual as far as medication, therapy, and treatment are concerned?
doctor
Answered by Dr. Ilir Sharka (17 hours later)
Brief Answer:
Nerve conduction studies are needed to evaluate the damage

Detailed Answer:
Hello again!

A peripheral nerve damage caused by a vitamin deficiency or a drug depends on the period of the drug intake or vitamin deficiency.

Usually if the drug is stopped or vitamin supplements are taken the damages are reversible.

If a long period is left without any interventions, the nerve damages can become irreversible.

Nerve conduction studies(electroneurography) are needed to evaluate the nerve damage and the possibility of damage reversal.

Hope to have been helpful!
