Suggest Treatment For Fever, Chills And Acute Tonsilitis

Question: Hello Dr ,
My son 14 yrs old . One month back he suffered from fever with chills and rigors. Came to know from GP that he has acute tonsilitis .He completed 5 days course of Co. Amoxiclave (augmentin)As soon as he stopped antibiotic , he got same symptoms
Fever , difficult in swallowing and sore throat .
Then again continued for more 5 days but again after 2 days had same syptoms and was feeling lathargic .
Then started phenoxybenzylpenicilin by Gp suspecting streptococcal .
He completed course for 7 days but after 2 days same symptoms .
Gp said to give her penicillin for 3 more days . Still giving .
Her tonsils still have little whitish spots.
Plz help me what to do as I am doctor as well.

Dear Sir

The whitish spots is likely to be due to follicular tonsillitis. As the symptoms are still persisting inspite of changing the antibiotic, possibility of resistant infection or atypical infection needs to be considered. I would suggest you the following

1. Swab taken from tonsill and pharynx : Routine microscopic examination and culture

2. Warm Saline gargling and Betadaine gargling : Alternate both of the following.

3. Change in antibiotic : After swab has been taken , in the meantime empirical treatment with fluroquinoline antibiotic like Levofloxacin will be helpful

Gp told me nothing will be found on throat swab.so he said leave it.I am in UK and can't get any antibiotic unless prescribed by GP though I am a Dr as well.could it be possible that might be fungal infection ??he feels so much lathargic otherwise he was a very active boy.Could you please tell me why his symptoms resolve when he takes antibiotics and comes back when stop the antibiotics .
Do you think it's streptoccoal as he had this infection when he was 7 yrs old and his follow up was done and echocardiography was done and it was ok.
Can I give him erythromycin instead of penicillin V .
Can't get those antibiotics which you prescribed.

Dear Sir

The very fact that it did respond to initiation of antibiotic rules out fungal infection as the cause. Fungal infection usually occurs in individual with low immunity.

Streptococcal infection usually respond to antibiotic already given . Possibility of either streptococcal infection which is resistant or atypical infection needs to be considered.

In my opinion getting a swab test will definitely help , even if its inconcusive it is worth(it will help in knowing the cause for recurrence )
Also If you have access to Erythromycin, you can put him on Erythromycin (it helps against atypical infection ).

Thanks so much Dr . I am quite satisfied with your diagnosis and treatment .
Wanted to ask few more questions
Is there fever in fungal infection. He feels so much week .
Is it dangerous to give too many antibiotics , can it lead to MRSA .
I am too much worried .
Should I give him more antibiotic like erythromycine .
Can I try anti fungal Nystatin drops ??
Does he need any blood tests ??
If so which are they ?

There is no fear of MRSA

Detailed Answer:
Dear Sir,

Fungal infection usually causes local symptoms like difficulty in swallowing or throat pain, it is unlikely to present with high grade fever. prolonged use of antibiotics may sometimes lead to fungal infection( as prolonged use of antibiotics will destroy the normal resident bacterial flora). But this kind of super added fungal infection usually causes only local symptoms, and unlikely to be associated with high grade fever or fever with cills/rigor.
Weakness is quite common after respiratory illness especially so if its viral infection.

I would suggest you the following

1. You can definitely use nystatin drops: If the white membrane or patch is due to fungal infection it will definitely respond( presence of white patch over tonsil apart from follicle or pseudomembrane, fungal infection needs to be considered)

2. Get a swab test done

3. after swab test Erythromycin can be epitically started

4. PhenoxyPenicillin is narrow spectrum antibiotic and less likely to be cause MRSA( at present MRSA is not a threat as effectively treating the symptoms)

5. Get Complete blood count along with Peripheral smear examination done: At present these two are sufficient

6. Multivitamin supplements

7. Make him sip a spoon of honey with 2 drops of fresh lemon juice added, to be done thrice a day.

8. Warm food like Chicken/tomato soup will be helpful

9. Preferable give warm water for consumption

10. Warm Saline gargling: To a glass of lukewarm water add 2 spoons of common table salt. make him gargle throat with it 3-4 times a day

Dear Dr XXXXXXX ,
Plz give me the dosage of Erythromycin 500 mg for a child of 14 yrs old and his weight is 58 kg.
One thing more to ask as he is having a hard blister on sole of his foot since 2 months .It is tender and he feels difficulty in walking sometimes blister gets bigger sometimes small but not going fully .
Is there any association with his recent fever .

Adult dosage can be given for him . Erythromycin 500 mg 8 th hourly.
The hard blister is likely to be infected corn over the sole of foot. It is no way related to present episode of fever .
