Suggest Treatment For Fever And Vomiting

Question: My son is suffering from fever since 2 days. He is 2 yrs old , weight=10kg. Doctor has prescribed 6.5 ml paracetamol TDS, 5ml seetus-P TDS and 2.5 ml of azithral 200 once a day (empty stomach).
1. After taking pcm and azithral in the morning he vomited in 5 minutes while having milk. Should I repeat the dose? (Today I didn't)
2. The fever is coming every 4 hrs. Even if it is not thr his head is hot.
3. I m giving pcm every 4 hrs, is it ok?

Hi...Coming to your queries directly -

1. usually any medicine vomited within 15 minutes should be repeated again.

2. Not only head will be hot, but abdomen or tummy also will be hot as the skin there had increased blood supply. So do not worry about localised heat.

3. Giving Paracetamol every 4th hourly is ok - provided the temperature is more than 100F. So check the temperature every 4th hourly and if it is more than 100F only you need to give. Otherwise you can wait.

1. If it is a viral, r the medicines mentioned above sufficient. There is only one antibiotic azithral 200 which is 2.5 ml OD.
Should any other antibiotic be given? Fever is always around 102-103. After fever it gets down to 100.5
2. He is vomiting after medicine almost every time. Should I give any vomiting medicine along with other medicines?

Hi...

1. Antibiotics are not required for viral illness at all. No need for azithromycin too as its an antibiotic.

2. To prevent vomiting, you can give syrup ondanisetron 2.5 ml thrice a day for 2 days. 30 minutes before other medication.

3. Regarding viral illness - If your kid is normal in between fever episodes in a single day...I don't think you should worry as its only 3rd day of fever. Danger signs include breathlessness/ rash/ low urine output/ seizures/ unconsciousness/ blood in motion. Don't worry if these are not there & the kid is active.

1. He is having a low urine output.
2. He is not eating anything at all. Just 1 or 2 biscuits, and milk once or twice. Otherwise also his appetite has decreased over last few months.
3. Another query is, if I put pediasure or any other health drink in his milk, he gets loose motions. (not just ow in viral, otherwise also.
Pls advice. I m worried for him as he is getting sick very frequently since last 6 months. Almost every month is repeated episode of fever or cough cold. He has become very weak. Weight has come down to 10 from 11 in last 6 months. I have done deworming also twice but no respite in his eating. (he does not eat junk food or outside food. No body in our house does.)
(He was born a pre-mature baby with a birth weight of 1.7 kg.)

Hi...I still feel that what your son is having a recurrent viral illness which is quite common in this age group.

I will tell you how a viral illness behaves. You can compare this description if it fits into your son's recurrent fevers.

Fever of few days without any localizing signs could as well a viral illness. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all. Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.

Coming to your queries -

1. If the kid is passing urine average quantity a minimum of 4 times per day - then it cannot be low urine out put.

2. At his age interest in play increases and interest in food decreases. This is quite natural. If he is active and playing around you need not worry. This age rather than the quantity of the food I suggest you concentrate more on the number of times you feed him and also make whatever you feed him calorie dense by adding good amount of ghee to it.

3. Sometimes high solute load in preparations like Pediasure can cause diarrhea or loose motions, it will settle as the age progresses.

You told he has recurrent cough and cold - kindly answer the questions below -

1. How many days per month does he cough or feel breathless?
2. How many nights per month does his sleep get disturbed due to above symptoms?
3. Does he feel breathless when he runs around or plays with other kids?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough always associated with fever?

Answers to Questions:
1. How many days per month does he cough or feel breathless?( Cold is more common than cough.)
2. How many nights per month does his sleep get disturbed due to above symptoms?( He generally sleeps a sound sleep. He gets up only when he has fever. He generally gets vomiting when there is high fever)
3. Does he feel breathless when he runs around or plays with other kids? (No)
4. Are the symptoms when there are seasonal changes? (Yes)
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.? (Yes, his grand father and great grand mother have asthma. )
6. Is the cough always associated with fever? (Most of the times.)

What I feel is:
1. He has a very low immunity.
2. He has lost his appetite.
I try to indulge him in physical activities & games and to give him fruits and salads to cure the above 2.
Can I do something else about it?

Coming to your queries -

1. This is not low immunity - this is just recurrent viral illness - do not worry.

2. Loss of appetite is not pathological if the kid is active otherwise and developmentally normal.

3. What you can do is -
a. Do not force feed as the kids develop aversion towards food.
b. Try to encourage him to feed with a spoon himself.
c. Initially try with foods he likes very much.
d. Rather than aiming at 3 large feeds - aim at 10 smaller feeds at 2
hourly intervals.

My suggestions -

1. It is not low immunity - this is allergy related problem./ it can be allergic rhinitis or seasonal allergy or viral associated wheeze.

2. He needs oral Monteleukast and Levosalbutamol oral or metered dose inhaler whenever he has severe cough.
